摘  要:目的总结应用双微导管技术行血管内治疗颅内不规则形复杂动脉瘤的初步经验,探讨其应用的可行性。方法回顾性分析2012年1月至2015年5月安徽省立医院神经外科收治的37例颅内不规则形复杂动脉瘤患者的临床资料。37例均采用双微导管技术行血管内治疗,其中单纯以双微导管技术治疗33例,支架辅助双微导管技术治疗4例。以格拉斯哥预后分级(GOS)评估患者预后。术后随访时间为6个月。结果37例患者行血管内治疗后即刻造影显示,动脉瘤致密栓塞35例,近全栓塞2例。所有患者均恢复良好(GOS分级V级),无并发症,无一例死亡。6个月后DSA随访24例,显示载瘤动脉通畅,动脉瘤无复发。13例失随访。结论双微导管技术对于颅内不规则形复杂动脉瘤疗效较好,具有可行性。Objectives To summarize the preliminary experience of endovascular treatment of irregular and complicated intracranial aneurysms with double microcatheter technique and to investigate the feasibility of its application. Methods From January 2012 to May 2015, the clinical data of 37 patients with irregular and complicated intracranial aneurysm admitted to the Department of Neurosurgery, Anhui Provincial Hospital were analyzed retrospectively. All 37 patients were treated endovascularly by using double microcatheter technique, 33 of them were treated with double microcatheter technique alone and 4 were treated with stent-assisted double microcatheter technique. Prognosis of patients were evaluated by the Glasgow outcome scale (GOS). The follow-up time after procedure was 6 months. Results Immediate angiography after procedure in 37 patients revealed that the aneurysms of 35 patients were densely embolized and 2 were near-totally embolized. All patients recovered well ( GOS grade V) , no complications and deaths occurred, After 6 months, 24 patients were followed up by DSA, and it revealed that the parent arteries were patent, and no recurrence of aneurysms was found. Thirteen patients were lost to follow-up. Conclusion The efficacy of double microcatheter technique for the treatment of irregular and complicated intracranial aneurysms is good and it has feasibility.
