无创呼吸机对高龄慢性心衰急性失代偿患者的疗效

摘　　要：目的:探讨无创呼吸机对高龄慢性心衰急性失代偿患者的疗效。方法:选择在我科住院的高龄慢性心衰急性失代偿患者196例,随机均分为常规治疗组和无创呼吸机组(在常规治疗组基础上接受无创呼吸机辅助治疗)。观察比较两组治疗前后总有效率、呼吸频率(RR)、心率(HR)、平均动脉压(MAP)、动脉血氧分压(PaO2)以及左室射血分数(LVEF)。结果:无创呼吸机组总有效率显著高于常规治疗组(90.82%比73.47%),P=0.007。治疗前两组上述各指标均无显著差异(P>0.05)。与治疗前比较,治疗后两组的RR、HR和MAP均显著下降,PaO2和LVEF显著升高(P<0.05或<0.01);与常规治疗组比较,无创呼吸机组治疗后RR[(27±6)次/min比(19±3)次/min],HR[(109±10)次/min比(73±8)次/min]和MAP[(110.5±9.0)mmHg比(81.6±8.0)mmHg]下降更显著,PaO2[(77±8)mmHg比(98±9)mmHg]和LVEF[(41.9±5.4)%比(52.7±5.5)%]升高更显著,P<0.05或<0.01。结论:高龄慢性心衰急性失代偿患者在常规治疗的基础上应用无创呼吸机治疗,能够有效缓解心功能衰竭的症状,改善呼吸频率、心率、平均动脉压、动脉血氧分压及左室射血分数等指标,改善心功能,值得推广。Objective：To explore therapeutic effect of noninvasive ventilator on advanced aged patients with acute decompensated chronic heart failure.Methods：A total of 196 advanced aged patients with acute decompensated chronic heart failure hospitalized in our department were randomly and equally divided into routine treatment group and noninvasive ventilator group（received auxiliary treatment with noninvasive ventilator based on routine treatment group）.Total effective rate,respiratory rate（RR）,heart rate（HR）,mean arterial pressure（MAP）,partial pressure of oxygen in artery（PaO2）and left ventricular ejection fraction（LVEF）were observed and compared between two groups before and after treatment.Results：Compared with routine treatment group,total effective rate（73.47% vs.90.82%）significantly rose in noninvasive ventilator group,P=0.007.There were no significant difference in above each indexes between two groups before treatment（P〉0.05）.Compared with before treatment,there were significant reductions in RR,HR and MAP,and significant rise in PaO2 and LVEF in both groups after treatment,P〈0.05or〈0.01;compared with routine treatment group after treatment,there were significant reductions in RR [（27±6）times/min vs.（19±3）times/min],HR [（109±10）beats/min vs.（73±8）beats/min]and MAP [（110.5±9.0）mmHg vs.（81.6±8.0）mmHg],and significant rise in PaO2 [（77±8）mmHg vs.（98±9）mmHg]and LVEF[（41.9±5.4）% vs.（52.7±5.5）%]in noninvasive ventilator group,P〈0.05or〈0.01.Conclusion：Noninvasive ventilator treatment based on routine therapy can effectively relieve symptoms of heart failure,improve RR,HR,MAP,PaO2,LVEF and cardiac function in advanced aged patients with acute decompensated chronic heart failure,which is worth extending.
