摘 要:目的:探究序贯式尿管对长期留置导尿患者预防泌尿系并发症的作用。方法:于2010年1月至2013年12月精神内科住院且需留置尿管3个月以上患者122例。随机数字表法将患者分为观察组和对照组。两组患者的尿管更换频率均为1次/2周,贮尿袋更换频率为1次/周,并配合治疗原发疾病。观察组序贯选用导管,对照组一直使用同样型号尿管,一般为16~18号导尿管。3个月后,观察两组患者漏尿及泌尿系统并发症的情况。结果:3个月治疗过程中,观察组发生漏尿现象的患者有3例,占4.92%;对照组患者发生漏尿现象的患者有13例,占21.31%,观察组发生漏尿现象的患者少于对照组(P〈0.05)。观察组患者发生尿道感染3例(4.92%),少于对照组患者的12例(19.67%);观察组患者发生尿道损伤患者1例(3.28%),少于对照组患者的8例(13.11%);观察组患者发生肾盂与输尿管积水患者0例(0.00%),少于对照组患者的12例(19.67%)。观察组患者发生泌尿系统并发症4例(6.56%),少于对照组患者的30例(49.18%)。结论:序贯选用尿管能够减少长期留置尿管患者发生漏尿及泌尿系并发症,值得临床应用及推广。Objective: To evaluate the effect of clinical application of sequential selection of catheter for the prevention of urological complications in patients with long- term catheterization. Methods: 122 cases of long- term catheterization admitted in Department of Psychiatry from January 2010 to December 2013 were included into the study. Patients were divided into the observation group and the control group,using a random number table. The frequency of changing catheter in the two groups was one time for every two weeks,and the frequency of changing urine collection bags was one time per week. Sequential catheter selection was used in the observation group,while in the control group the same size of catheter( generally 16 ~ 18) was selected to change each time. Cases of urine leakage and urinary system complications were observed and recorded.Results: After 3 months,there were 3 cases of urine leakage( 4. 92 %) in the observation group,significantly less than 13 cases( 21. 31 %) in the control group. The number of patients with urethral infection in the observation group was 3( 4. 92 %),significantly less than 12 cases( 19. 67%) in the control group. 1 case with urethral injury was in the observation group( 3. 28 %),significantly less than 8 cases( 13. 11 %) in the control group. There was no cases with hydronephrosis and hydroureter in the observation group,which was significantly less than 12 cases( 19. 67 %)in the control group. The number of patients with complications of urinary system in the observation group was 4( 6. 56 %),less than 30 cases( 49. 18 %) in the control group. Conclusion: Sequential selection of catheter can reduce the leakage and urinary complications in patients with long- term indwelling urinary catheter. It is worthy of clinical application and promotion.
