2006-2014年玉溪市3326例新发现HIV/AIDS首次CD4^＋T淋巴细胞检测率和及时率分析

摘　　要：目的了解2006-2014年玉溪市新发现艾滋病病毒感染者/艾滋病病人(HIV/AIDS)病例首次CD4^+T淋巴细胞计数检测率和检测及时率及其影响因素。方法使用艾滋病综合防治数据信息系统中的病例报告卡数据库和随访数据库以及历年CD4^+T淋巴细胞检测原始记录,截止日期为2015年6月,对相关数据进行统计分析。结果 2006-2014年间,玉溪市共新报告3 326例HIV/AIDS,其中有69.66%的HIV/AIDS获得CD4^+T淋巴细胞计数检测,36.62%的HIV/AIDS是在2周之内获得了检测结果,历年首次CD4^+T淋巴细胞检测率和及时率分别为为51.72%,47.38%,63.50%,66.82%,74.50%,84.44%,78.46%,75.17%,69.17%(=125.46,P<0.001)和4.21%,8.31%,18.99%,41.18%,45.61%,59.08%,47.69%,37.36%,46.64%(=292.524,P<0.001),均有上升趋势。交通不便和防治艾滋病人力资源不足等多种因素影响CD4^+T淋巴细胞检测率和及时率。结论必须提高新发现HIV/AIDS病例的CD4^+T淋巴细胞计数检测率以及检测及时率,以推动符合抗病毒治疗要求的病例的及时转介接受抗病毒治疗,确保完成"艾滋病防治一站式服务"。Objective This work was to understand the relevant impact factors of the testing rate and its timeliness of CD4 ＋ T cell counts of 3326 newly reported HIV cases in Yuxi prefecture,2006- 2014. Methods Date were collected with the HIV/AIDS Case Reporting Cards,Follow- up Cards and original record by the end of may,2015. The data were then statistically analyzed. Results Among the 3326 newly reported HIV/AIDS cases between 2006 and 2014,69. 66% received CD4 ＋ T cell counts testing and 36. 62% received results within two week. The testing rate and timeliness for each year were 51. 72%,47.38%,63. 50%,66. 82%,74. 50%,84. 44%,78. 46%,75. 17%,69. 17%（ = 125. 46,P 0. 001） and 4. 21%,8.31%,18. 99%,41. 18%,45. 61%,59. 08%,47. 69%,37. 36%,46. 64%（ = 292. 524,P 0. 001）,respectively,all with upward trend. Univariate analysis showed that the testing rate and timeliness of CD4 ＋ T cell counts were affected by factors such as traffic and human resource. Conclusion The testing rate and timeliness of CD4 ＋ T cell counts of newly reported HIV cases must be increased to promote timely referral of patients for antiretroviral treatment,so as to achieve the one- stop service in HIV/AIDS treatment.
