摘 要:目的探讨超声弹性成像与二维超声在肝脏病灶良恶性病变中的应用价值.方法 2012年6月至2014年6月选取84例经病理组织确诊为肝脏病病变的患者为研究对象,术前对患者行超声弹性成像及常规超声检查,并将影像学诊断结果与病理手术结果进行对比.结果 84例肝脏病灶良恶性病变患者中良性病变38例(68个病灶),恶性肿瘤46例(82个病灶).二维超声诊断肝脏良、恶性病变准确率为76.47%、82.93%,灵敏度分别为70.25%、68.23%,特异度分别为62.45%、72.32%.超声弹性成像诊断良、恶性肿瘤准确率为91.17%、95.12%,灵敏度分别为93.65%、90.25%,特异度分别为80.25%、88.92%,差异有统计学意义(P<0.05).与良性病变相比,恶性病变组弹性成像分级较高,差异有统计学意义(P<0.05).结论超声弹性成像技术为肝脏病变诊断及鉴别提供了全新的方法,提高了肝脏良恶性病变的诊断准确性及灵敏性.Objective To investigate the application value of ultrasound elastography and two-dimensional ultrasound in benign and malignant lesions of liver. Methods 84 patients with liver disease were selected as research objects who underwent preoperative ultrasound elastography and conventional ultrasonography from June2012 to June 2014. A comparison was made between the results of imaging diagnosis and surgical pathology. Results There were 38 cases(68 lesions) of benign malignant liver lesions and 46 cases of malignant tumors(82lesions). For benign and malignant lesions, the diagnostic accuracy, sensitivity and specificity of dimensional ultrasound diagnosis were 76.47% and 82.93%,70.25% and 68.23%,and 62.45% and 72.32%,respectively.However, the diagnostic accuracy, sensitivity and specificity of ultrasound elastography were 91.17% and95.12%, 93.65% and 90.25%,and 80.25% and 88.92%, respectively. There were the statistically significant differences between the two methods(P〈0.05). Compared with benign lesions,malignant lesions showed higher grading using ultrasound elastography, with a statistically significant difference(P〈0.05). Conclusion Ultrasound elastography technique provides a new method to diagnose the liver diseases,which can improve the accuracy and sensitivity of the diagnosis of benign and malignant lesions.
