摘 要:目的:观察大鼠用不同剂量阿奇霉素灌胃后,胃排空时间及血浆胃动素浓度的变化,探讨阿奇霉素致胃肠道症状的可能机制。方法:80只SD大鼠随机分为10组,对照组、小剂量1 d组、小剂量3d组、小剂量6d组、中剂量1d组、中剂量3d组、中剂量6d组、大剂量1d组、大剂量3d组、大剂量6d组;观察组大鼠用阿奇霉素灌胃,小剂量组、中等剂量组、大剂量组分别为3mg/100g、6mg/100g、12mg/100g。所有大鼠先用SPECT行胃排空检测,试餐为含有^(99m)Tc-DTPA的牛奶,用专用软件分析原始数据,得到全胃半排时间(GET1/2)、50min末残留率、胃排空曲线。胃排空检测结束后,经腹主动脉采集动脉血2ml,运用放免法测定血浆胃动素浓度。结果:小剂量组、中剂量组、大剂量组全胃半排时间明显缩短,与正常对照组(64.21±7.24)min比较,差异有统计学意义,P<0.05。组内不同时间点比较无统计学意义,P>0.05。观察组血浆胃动素浓度,小剂量组与正常对照组比较无统计学意义,P>0.05;中剂量组、大剂量组与正常对照组比较明显增高,差异有统计学意义,P<0.05。结论:阿奇霉素可促进胃排空。阿奇霉素促进胃排空的机制可能为小剂量下仅为胃动素样作用;中剂量、大剂量时除胃动素样作用外,有内源性胃动素参与。Objective: To investigate the effects of azithromycin on gastric emptying and plasma motilin in rats, thus exploring mechanism of azithromycin to Gastrointestinal Symptom. Methods:There were 10 groups randomly dividing in eighty Sprague-Dawley(SD) rats, i.e, the normal control group, small doses 1 day group, small doses 3 day group, small doses 6 day group, middle doses 1 day group, middle doses 3 day group, middle doses 6 day group, large doses 1 day group, large doses 3 day group, large doses 6 day group, 8 in each group. The rats of observer groups were gastrogavaged by azithromycin. The dosage were 3mg/100 g, 6mg/100 g and 12mg/100 g, respectively. First, gastric emptying was measured in unanesthetized rats by single photon emission computed tomography(SPECT) after gavaged milk loaded with technetium-99 m diethylenetriamine pentaacetic acid((99m)Tc-DTPA), the gastric half-emptying time were calculated. Subsequently, blood was collected from the abdominal aorta to measure plasma motilin. Results:GET1/2 were more significantly decreased in observer groups than in control groups. There was no significant in concentration of motilin in small dose groups compared to control groups. The plasma motilin of latter groups were significantly increased compared to control groups, P〈0.05. Conclution: Azithromycin could promote gastric emptying. Not only motilin-like effection, but also endogenous motilin participation took part in the mechanisms of promoting gastric emptying.
