摘 要:目的探讨芪丹通络颗粒对糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)患者神经传导速度及丙二醛(malondialdehyde,MDA)、超氧化物歧化酶(superoxide dismutase,SOD)活力和总抗氧化(total antioxidant capacity,TAOC)的影响。方法选取2012年6月~2014年3月来大同市第三人民医院内分泌科就诊的DPN患者69例,采用随机数字法分为治疗组(n=35)与对照组(n=34),治疗组采用芪丹通络颗粒联合甲钴胺注射液治疗,对照组仅给予甲钴胺注射液。观察比较2组患者临床疗效、正中神经及腓神经的感觉神经传导速度(sensory nerve conduction velocity,SCV)及运动神经传导速度(motor nerve conduction velocity,MCV)、血清及肝组织中MDA,SOD和TAOC的水平。结果治疗组TCSS评分显著降低(P〈0.05),且与对照组比较差异有统计学意义(P〈0.05)。治疗后,治疗组疗效明显高于对照组(Z=2.205,P=0.027)。正中神经和腓总神经的MCV及SCV均显著改善(P〈0.05),但2组间比较差异无统计学意义。与对照组比较,治疗组MDA显著降低,SOD、TAOC显著升高(P〈0.05),TCSS评分显著降低(P〈0.05)。结论芪丹通络颗粒可以明显提高DPN患者的疗效,并改善患者的临床症状和氧化应激水平。Objective To study the effect of Qidantongluo particles on nerve conduction velocity and the levels of MDA,SOD and total antioxidant capacity( TAOC) in diabetic peripheral neuropathy patients. Methods 69 cases of diabetic peripheral neuropathy patients in our hospital from June 2012 to March 2014 were divided into treatment group( 35 cases) and control group( 34 cases) by random number method. Qidantongluo particles combined with mecobalamin injection in treatment group and only mecobalamin injection in control treatment. The clinical therapeutic effect sensory nerves conduction velocity( SCV),motor nerve conduction velocity( MCV),MDA and SOD levels in serum and liver tissue and TAOC between two groups were compared. Results The TCSS score post-treatment in treatment group decreased significantly( P〈0. 05),and had significant difference compared with control group( P〈0. 05). The total effective rate in treatment group was higher than control group( Z = 2. 205,P = 0. 027). After treatment,the MCV and SCV of the median nerve and peroneal nerve in two groups had a significant improvement( P〈0. 05),but there were no significant difference between two groups. Compared with contol group,the MDA level decreased and SOD level,TAOC increased,TCSS score decreased significantly in treatment group( P〈0. 05). Conclusion MDA Qidantongluo particles could obviously improve clinical symptoms and levels of oxidative stress in DPN patients.
