摘 要:目的:探讨奥美拉唑与甲氨蝶呤药物相互作用的机制和临床特点,为临床合理用药提供参考。方法:分析和评价临床1例9岁男孩发生奥美拉唑与甲氨蝶呤药物相互作用致甲氨蝶呤排泄延迟的病例,并以"质子泵抑制剂、甲氨蝶呤"为关键词,检索1990-2015年间中国期刊网全文数据库(CNKI)、万方数据库及Pubmed数据库的相关文献,进行统计分析。结果:该类药物相互作用的个案报道仅检索到10篇,其中2篇为国内报道。奥美拉唑可能延缓甲氨蝶呤的排泄,建议避免两者合用。结论:临床应高度重视奥美拉唑与甲氨蝶呤的药物相互作用,加强临床观察,确保用药安全。Objestive: To discuss the mechanisms and clinical features of interaction between omeprazole and methotrexate( MTX) in order to provide references for rational use of proton pump inhibitors( PPIs) and MTX.Methods: One case of decreased elimination of MTX potentially induced by a drug interaction between methotrexate and omeprazole in a 9-year-old boy was analyzed and evaluated. Published literatures were searched for cases reports of interaction between PPIs and MTX through CNKI,Wan Fang Database,and Pubmed Database from 1990 to2015. Results: Only 10 case reports were collected including two in China. Omeprazole may delay MTX elimination; therefore,when prescribing MTX,an alternative to omeprazole should be considered. Conclusion: Physicians should be alerted to this potential drug interaction in patients receiving concomitant PPIs and high-dose methotrexate.
