Association between the Insertion/Deletion Polymorphism of ACE Gene and Athlete's Sport Performance in Endurance Sports:A Meta-analysis

摘  要:目的系统评价血管紧张素转换酶(ACE)基因插入/缺失(I/D)多态性与耐力型运动的关联性。方法计算机检索Pub Med、EMbase、CNKI、CBM、VIP和Wan Fang Data数据库,搜集关于ACE基因I/D多态性与耐力型运动能力相关性的病例-对照研究,检索时限均为建库至2015年8月1日。由2位评价者独立筛选文献、提取资料并评价纳入研究的偏倚风险后,采用Rev Man 5.3软件进行Meta分析。结果最终纳入37个病例-对照研究,共计3 032例运动员和10 857例对照。Meta分析结果显示:ACE基因I/D多态性与运动员耐力型运动能力相关[DD+DI vs.II:OR=0.75,95%CI(0.67,0.83),P<0.01;DD vs.II:OR=0.73,95%CI(0.61,0.87),P<0.01;DI vs.II:OR=0.74,95%CI(0.66,0.83),P<0.01;D vs.I:OR=0.85,95%CI(0.77,0.94),P<0.01]。进一步分析发现,仅男性运动员耐力型运动能力与ACE基因I/D多态性存在关联性[DD+DI vs.II:OR=0.73,95%CI(0.61,0.88),P<0.01;DD vs.II:OR=0.75,95%CI(0.60,0.93),P=0.01;DI vs.II:OR=0.70,95%CI(0.60,0.93),P<0.01;D vs.I:OR=0.87,95%CI(0.77,0.97),P=0.01]。按人种亚组分析结果显示:在高加索人种中,除DD vs.DI+II遗传模型外,其余遗传模型均与运动员耐力型运动能力相关[DD+DI vs.II:OR=0.74,95%CI(0.65,0.84),P<0.01;DD vs.II:OR=0.72,95%CI(0.58,0.90),P<0.01;DI vs.II:OR=0.73,95%CI(0.64,0.84),P<0.01;D vs.I:OR=0.87,95%CI(0.81,0.94),P<0.01];在非洲人种中,DD vs.DI+II[OR=0.75,95%CI(0.57,0.98),P=0.04]、DD vs.II[OR=0.62,95%CI(0.42,0.92),P=0.02]和D vs.I[OR=0.80,95%CI(0.66,0.96),P=0.02]遗传模型与运动员耐力型运动能力相关;在亚洲人种中,5种遗传模型与运动员耐力型运动能力均无关联。结论现有证据表明,ACE基因I/D多态性与总体人群、男性人群,以及种族亚组中高加索人群的耐力型运动能力存在关联性,其中ACE基因的D等位基因与耐力型运动能力呈负相关,I等位基因与耐力型运动能力呈正相关。受纳入研究的质量的限制,上述结论仍需开展更多高质量的病例-对照研究或队列研究Objective To systematically review the association between the insertion/deletion(I/D) polymorphism of angiotension-converting enzyme(ACE) gene and the athletes' performance in endurance sports. Methods Databases including Pub Med, EMbase, CNKI, CBM, VIP, and Wan Fang Data were searched up to August 1^(st), 2015 to collect casecontrol studies about the association between ACE I/D polymorphism and the athletes' performance in endurance sports. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Then, meta-analysis was performed using Rev Man 5.3 software. Results A total of 37 case-control studies involving 3 032 athletes and 10 857 controls were included. The results of meta-analysis showed that significant association was found between ACE I/D polymorphism and the athletes' performance in endurance sports(DD+DI vs. II: OR=0.75, 95%CI 0.67 to 0.83, P0.01; DD vs. II: OR=0.73, 95%CI 0.61 to 0.87, P0.01; DI vs. II: OR=0.74, 95%CI 0.66 to 0.83, P0.01; D vs. I: OR=0.85, 95%CI 0.77 to 0.94, P0.01). Specifically, the ACE I/D polymorphism was significantly associated with the performance of male athletes in endurance sports(DD+DI vs. II: OR=0.73, 95%CI 0.61 to 0.88, P0.01; DD vs. II: OR=0.75, 95%CI 0.60 to 0.93, P=0.01; DI vs. II: OR=0.70, 95%CI 0.60 to 0.93, P0.01; D vs. I: OR=0.87, 95%CI 0.77 to 0.97, P=0.01). Subgroup analysis of ethnicity showed that, in Caucasians, except for genetic model DD vs. DI+II, the other 4 genetic models were significantly associated with the athletes' performance in endurance sports(DD+DI vs. II: OR=0.74, 95%CI 0.65 to 0.84, P0.01; DD vs. II: OR=0.72, 95%CI 0.58 to 0.90, P0.01; DI vs. II: OR=0.73, 95%CI 0.64 to 0.84, P0.01; D vs. I: OR=0.87, 95%CI 0.81 to 0.94, P0.01); in Africans, significant associations with the athletes' performance in endurance sports were found in genetic model DD vs. DI+II(OR=0.75, 95%CI 0.57 to 0.98, P=0.04), genetic model DD vs. II
