血清脂蛋白相关磷脂酶A2水平与冠状动脉粥样硬化性病变的关系

摘　　要：目的探讨血清脂蛋白相关磷脂酶A2（Lp-PLA2）水平与冠状动脉（冠脉）粥样硬化性病变的关系。方法行冠脉造影患者327例,224例诊断为冠心病（CHD组）,103例无CHD作为对照组。检测并分析血清Lp-PLA2水平与CHD病变的关系。结果 CHD组Lp-PLA2水平高于对照组[（134.74±45.67）ng/ml vs.（114.38±45.80）ng/ml]（P〈0.01）。经调整年龄、糖尿病、HDL-C和收缩压等因素,Lp-PLA2水平与CHD密切相关（OR=1.262,95%CI：1.160-1.610,P〈0.05）。CHD组Lp-PLA2水平与冠脉病变支数和Gensini积分呈正相关（r=0.190和0.222,P〈0.01）。结论血清Lp-PLA2水平是CHD的独立危险因素,可提示冠脉病变的血管狭窄程度,但与病变稳定性无密切相关。Objective To investigate the relationship of serum lipoprotein-associated phospholipase A2（Lp-PLA2）and the lessions of coronary arterial atherosclerosis.Methods Coronary angiography was performed in 327 consecutive patients,of whom 158 cases were diagnosed as coronary heart disease（group CHD）and 103 cases were not as the controls（group C）.Serum levels of Lp-PLA2 were measured and the relationship of serum Lp-PLA2 and the lessions of coronary arterial atherosclerosis was analyzed.Results Serum level of Lp-PLA2 in group CHD was higher than that in group C[（134.74±45.67）ng/ml vs.（114.38±45.80）ng/ml]（P〈0.01）.After adjusted for age,blood pressure,diabetes and high density lipoprotein cholesterol,serum level of Lp-PLA2 was correlated with CHD（OR=1.262,95%CI：1.160-1.610,P〈0.05）.The level of Lp-PLA2 was positively correlated with the number of diseased coronary branches and Gensinis scores（r=0.190 and 0.222,P〈0.01）.Conclusion Serum Lp-PLA2 is an independent risk factor for CHD,which may indicate the severity of coronary arterial atherosclerosis,but is not closely related to the stability of the lesions.
