摘　　要：目的：探讨Supreme喉罩（SLMA）在强直性脊柱炎（AS）全髋关节置换手术患者中的应用效果。方法：选择26例（36髋）在全麻下行全髋关节置换手术患者,麻醉诱导后2 min侧卧位下行喉罩插入,记录患者诱导前（T_0）、诱导后（T_1）、喉罩置入即刻（T_2）、置入2 min（T_3）、置入4 min（T_4）、喉罩拔除即刻（T_5）以及拔除2 min（T_6）的收缩压（SBP）、舒张压（DBP）、平均动脉压（MAP）、血氧饱和度（SpO_2）及心率（HR）;观察记录喉罩置入的次数、时间及成功率;术后随访有无牙齿松动、咽喉疼痛、声音嘶哑及吞咽困难等不良反应。结果：本组26例患者均一次成功置入喉罩。与T_0比较,T_1~T_6的SBP、DBP明显下降;而T_1~T_6时点的SBP、DBP及HR均处于平稳状态,未出现明显波动,血流动力学稳定。喉罩置入时间为（1.2±0.5）min;手术过程中所有患者SpO_2均保持在96%以上,气道阻力正常。除术后随访2例患者出现轻微咽喉疼痛,其他病例未见返流误吸等并发症发生。结论：SLMA应用在AS全髋关节置换手术患者,操作简单、插管成功率高,速度快,并发症少。Objective： To analyze application results of Supreme（Supreme laryngeal mask airway, SLMA） laryngeal mask in ankylosing spondylitis patients with total hip replacement surgery. Methods： Chose general anesthesia total hip replacement surgery 26 patients（36 hips）, after induction of anesthesia 2min lateral position downstream laryngeal mask insertion, patients before induction（T_0） record, after induction（T_1）, LMA insertion immediately（T_2）, placed 2 min（T_3）, placed 4min（T_4）, the immediate removal of the laryngeal mask（T_5）, removal of 2 min（T_6） of SBP, DBP, MAP, SpO_2, HR; the number of LMA insertion of records time and success rate; Postoperative follow-up whether the loose teeth, sore throat, hoarseness, difficulty swallowing adverse reactions. Results： The group of 26 patients was successfully implanted a laryngeal mask. Compared with T_0, T_1~T_6 of SBP, DBP decreased; and T_1~T_6 point of SBP, DBP, HR are in a steady state, does not appear significant fluctuations in hemodynamic stability. Intubation（LMA insertion） time was 1.2±0.5 min. SpO 2 during surgery all patients were maintained at 96% and normal airway resistance. In addition to two patients were followed up for a slight sore throat, no other cases of regurgitation and aspiration and other complications. Conclusion： SLMA in ankylosing spondylitis patients undergoing total hip replacement application, simple operation, intubation success rate, faster, less complication.
