摘 要:目的探讨氯沙坦和环磷腺苷葡胺联合使用对肺心病患者肺部功能和免疫功能的影响。方法选择2014年3月至2015年3月肺心病患者124例,按照随机分组原则分为实验组和对照组,每组62例。给予对照组患者常规治疗,在相同常规治疗的基础上给予实验组患者口服氯沙坦和静脉滴注环磷腺苷葡胺治疗。测定患者治疗前后心功能指标,包括左心室射血分数(LVEF)、心输出量(CO)和每搏量(SV);测定患者治疗前后肺功能指标,包括一秒用力呼气容积(FEV1)、用力肺活量(FVC);测定患者治疗前后外周血中Ig A、Ig G和补体C3含量变化,评价免疫功能。结果两组患者治疗前LVEF、CO和SV无显著差异(P>0.05);1个疗程后,实验组三项指标增加量显著高于对照组(P<0.05)。两组患者治疗前FEV1和FEV1/FVC无显著差异(P>0.05);1个疗程后,实验组两项指标增加量显著高于对照组(P<0.05)。两组患者治疗前Ig A、Ig G和补体C3含量无显著差异(P>0.05);1个疗程后,实验组患者补体C3含量显著增加(P<0.05)。结论氯沙坦和环磷腺苷葡胺联合治疗肺心病可以有效提高患者心肺功能,改善肺组织损伤,增加体内补体C3含量。Objective To investigate the effects of losartan and cyclic adenosine phosphate on pulmonary function and immune function in patients with pulmonary heart disease. Methods 124 patients with pulmonary heart disease were randomly divided into control and experimental groups. Conventional treatment was applied to control group. In addition to conventional treatment,losartan and intravenous infusion of adenosine were applied to experimental group. Cardiac function indexes were measured,including left ventricular ejection fraction( LVEF),cardiac output( CO) and stroke volume( SV). Pulmonary function indexes including 1 second forced expiratory volume( FEV1) and forced vital capacity( FVC); C3,Ig G and Ig A levels in peripheral blood of patients were detected before and after treatment. Results There were not significant differences in LVEF,CO and SV before treatment( P〈0. 05). After the treatment,the three indexes in experimental group were significantly higher than those in control group( P〈0. 05). There were not significant differences in FEV1 and FEV1 / FVC before the treatment( P〉0. 05). After the treatment,the two indexes in experimental group were significantly higher than those in control group( P〈0.05). There were not significant difference in Ig A,Ig G and C3 levels before the treatment( P〉0. 05). After the treatment,the C3 level in experimental group was significantly higher than that in control group( P〈0. 05). Conclusions The combination of losartan and cyclic adenosine phosphate in the treatment of pulmonary heart disease could effectively improve heart and pulmonary function,and increase C3 content.
