围麻醉期心理干预对老年患者术后镇痛及胃肠功能恢复的影响

摘　　要：目的:探讨围麻醉期心理干预对老年患者硬膜外自控镇痛及胃肠功能恢复的影响。方法:选择60例下腹部手术老年患者,术前采用随机法分组,干预组和对照组各30例;干预组进行围麻醉期心理干预,对照组按常规做术前访视;术后干预组5 mg吗啡,对照组10 mg吗啡硬膜外自控镇痛,观察患者各时间点镇痛评分、镇静评分,肠鸣音恢复时间、肛门第一次排气情况、及不良反应发生情况。结果:干预组与对照组比较,镇痛评分差异无统计学意义(P>0.05),镇静评分明显低于对照组,差异有统计学意义(P<0.01);肠鸣音恢复、肛门第一次排气时间比较,差异有统计学意义(P<0.05);不良反应发生情况(恶心、呕吐、皮肤瘙痒等)发生率少。结论:围麻醉期心理干预能增强老年患者术后镇痛效果,同时能促进术后胃肠功能的恢复。Objective To study the effect of psychological intervention on postoperative analgesia and gastrointestinal functional recovery during anesthesia. Method sixty elderly patients with lower abdominal surgery were seledcted. Preoperative stochastic method was used to divide into intervention group 30 cases and control group 30 cases; Intervention group was given around the time of anesthesia psychological intervention,the control group routinely did preoperative visits; Postoperative intervention group was given 5 mg morphine,the control group 10 mg morphine epidural controlled analgesia,the patients＇ pain score at each time point,sedation score,bowel sounds recovery time,anus exhaust situation,for the first time and adverse reactions occur were observed. Results Intervention group and control group analgesic scores had no significant difference（ P〈0. 05）,sedation scores were significantly lower than the control group（ P〈0. 01）; Bowel sounds recovery,anal exhaust time for the first time were significant differences between the two groups（ P〈0. 05）; the incidence rate of adverse reaction（ nausea,vomiting,itching,etc.） was low. Conclusion Psychological intervention during anesthesia can strengthen elderly patients postoperative analgesia effect,also can promote the recovery of gastrointestinal function after surgery.
