微创下小切口卵圆钳辅助穿针修复急性跟腱断裂远期预后观察

摘　　要：目的探讨微创下小切口卵圆钳辅助穿针修复急性跟腱断裂患者的远期预后。方法选择2010年1~11月保定市第二医院接诊的40例急性跟腱断裂患者为研究对象,按随机数字表法分为观察组和对照组,各20例。观察组采用微创下小切口卵圆钳辅助穿针修复方式进行治疗,对照组采用常规的切口Krackow的缝合方法进行治疗。比较两组患者Arner Lindholm功能评分以及患者术后并发症发生情况。结果观察组患者术后1年Arner Lindholm功能评分优良率高于对照组[95.0%（19/20）比70.0%（14/20）],差异有统计学意义（P〈0.05）。观察组患者术后总并发症发生率低于对照组[5.0%（1/20）比35.0%（7/20）],差异有统计学意义（P〈0.05）。结论微创下小切口卵圆钳辅助穿针修复方式治疗急性跟腱断裂患者恢复快,并发症发生率低,可提高患者的生活质量,值得在临床上推广。Objective To investigate the long-term prognosis of minimally invasive incision oval clamp auxiliary needle repair in patients with acute achilles tendon rupture. Methods Total of 40 patients with acute achilles tendon rupture in Baoding Second Hospital were selected from Jan. 2010 to Nov. 2010. The patients were divided into control group and observation group according to random number table method,20 cases each. The observation group was given minimally invasive small incision oval clamp auxiliary needle restorative treatment,while the control group was treated by conventional incision Krackow method. Arner Lindholm function scores and complications after surgery in the two groups were compared. Results One year after surgery the good rate of Arner Lindholm function score in the observation group was higher than the control group[95. 0%（ 19 /20） vs 70. 0%（ 14 /20） ],a significant difference between the two groups（ P〈 0. 05）. Postoperative complication rate in the observation group was significantly lower than the control group[5. 0%（ 1 /20） vs 35. 0%（ 7 /20） ],a significant difference between the two groups（ P〈 0. 05）.Conclusion The small incision minimally invasive restorative oval clamp auxiliary needle treatment of acute achilles tendon rupture has the advatages of faster recovery,lower incidence of complications and improvement of the quality of life of the patients,thus is worth of promotion in clinical.
