中枢神经感染患者血脑屏障通透性与脑脊液IgG的相关性分析

摘　　要：目的探讨中枢神经感染患者血脑屏障通透性与脑脊液IgG指数、IgG合成率的相关性,为临床诊断和治疗提供参考依据。方法选取2013年2月-2014年3月医院中枢神经系统感染性疾病住院患者56例为研究对象,其中结核性脑膜炎22例(A组)、化脓性脑膜炎15例(B组)、病毒性脑膜炎19例(C组),同期选取21例有腰椎穿刺指征并排除中枢神经系统感染患者为对照组(D组),抽取其脑脊液和血清标本,进行检验和对比,数据采用SPSS 17.0软件进行统计分析。结果在脑脊液IgG及IgG比值的比较中,A、B、C组患者的IgG分别为(155.34±25.37)(146.21±37.45)(107.45±26.44)mg/L;IgG比值分别为(18.56±5.45)(17.45±4.67)(17.78±4.12)×10-3,均比D组患者IgG及IgG比值高,差异有统计学意义(P<0.05)。结论中枢神经感染患者血脑屏障通透性与脑脊液IgG指数、IgG合成率存在一定的相关性,需综合考虑IgG、IgG指数及IgG合成率等指标。OBJECTIVE To investigate the relationship between the permeability of blood brain barrier and the cerebrospinal fluid IgG index and IgG synthesis rate in the patients with central nervous system infection so as to provide guidance for clinical diagnosis and treatment.METHODS A total of 56 patients with central nervous system infectious diseases who were treated in the hospital from Feb 2013 to Mar 2014 were recruited as the study objects and divided into the tubercular meningitis group with 22cases（the group A）,the suppurative meningitis group with 15cases（the group B）,and the viral meningitis group with 19cases（the group C）,meanwhile,21 patients who had the lumbar puncture indications and were without central nervous system infections were assigned as the control group（the group D）.The cerebrospinal fluid and serum specimens were collected and cultured,and the statistical analysis of data was performed with the use of SPSS 17.0software.RESULTS The level of cerebrospinal fluid IgG was（155.34±25.37）mg/L in the group A,（146.21±37.45）mg/L in the group B,（107.45±26.44）mg/L in the group C;the IgG ratio was（18.56±5.45）×10^-3 in the group A,（17.45±4.67）×10^-3 in the group B,（17.78±4.12）×10^-3 in the group C;the level of IgG and IgG ratio of the three groups were higher than those of the group B（P〈0.05）.CONCLUSION The permeability of blood brain barrier of the patients with central nervous system infection is associated with the cerebrospinal fluid IgG index and the IgG synthesis rate,and it is necessary to comprehensively consider the indicators such as the IgG,IgG index,and IgG.
