摘 要:目的分析经皮椎间盘内医用臭氧注射治疗腰椎间盘突出症围术期不采用抗菌药物预防感染的可行性,为临床治疗提供参考依据。方法选取2014年1-12月骨科收治的腰椎间盘突出症患者98例,随机分为对照组与研究组,每组各49例,两组患者均采用经皮椎间盘内医用臭氧注射治疗,研究组术前不给予抗菌药物预防感染,对照组术前30min静脉滴注1.5g头孢呋辛钠,对比两组患者术后并发症发生率,数据采用SPSS 13.0软件进行统计分析。结果研究组患者术后发生椎间盘感染1例,感染率2.04%;对照组患者发生椎间盘感染2例,感染率4.08%;两组患者感染率比较差异无统计学意义;对照组患者术后恢复为优的占53.06%,明显高于研究组的30.61%,差异有统计学意义(P<0.05);两组患者血白细胞、C-反应蛋白和中性粒细胞升高阳性率比较,差异均无统计学意义;手术后对照组和研究组患者并发症总发生率差异无统计学意义。结论经皮椎间盘内医用臭氧注射治疗腰椎间盘突出症具有较好的安全性,围术期不采用抗菌药物预防感染的可行性较高。OBJECTIVE To analyze the feasibility of using ozone injection in the treatment of patients with lumbar disc herniation without use of antibiotics to prevent infections.METHODS Totally 98 cases of patients with lumbar intervertebral disc herniation admitted from Jan.2014-Dec.2014 were included in the study.They were randomly assigned to control group and study group,with 49 cases in each group.Both groups of patients received percutaneous intradiscal ozone injection for the treatment.Before operation,study group was not given antibiotics to prevent infections,while control group was given 1.5g intravenous injection of cefuroxime sodium injection half an hour before operation.The incidences of complications were compared between the two groups.Data were analyzed with SPSS 13.0.RESULTS After operation,there was 1case of lumbar disc infection in study group,infection rate was 2.04%;while there were 2cases of lumbar disc infection in control group,and infection rate was4.08%.The difference of infection rates was not significant.In control group,the effective recovery rate of patients after operation was 53.06%,significantly higher than that of the study group 30.61%(P〈0.05).There was no significance in the elevation of positive rates of leukocyte,C-reactive protein and neutrophil between the two groups.The difference of incidences of complication in control group and study group was not significant.CONCLUSIONPercutaneous intradiscal ozone injection is secure in the treatment of lumbar disc herniation,and is of high feasibility for preventing infections during peri-operation period.
