抗菌药物门诊处方临床应用分析

摘　　要：目的分析医院门诊处方抗菌药物临床使用情况及其使用过程中存在的问题,为提高临床用药的合理性提供参考依据。方法随机抽取2013年1月-12月6 000份处方,对抗菌药物的应用比例、用药指征、种类、联合用药和给药途径进行分析,数据采用SPSS 17.0软件进行统计分析。结果抗菌药物处方有1 867份,占31.12%,其中不合理处方206份,包括无适应证用药、遴选不适宜、用法用量不适宜和其他;联合用药包括单一用药1 316份占70.49%、二联用药406份占21.75%,≥三联145份占7.76%。结论医院抗菌药物的临床应用存在无适应证用药、所占比例高、联合用药不当、给药途径不合理及药物选择欠佳等问题,应继续加强抗菌药物的使用管理。OBJECTIVE To analyze the clinical use of antimicrobial agents in outpatient prescription of our hospital and find out the problems in the application of antimicrobial agents so as to provide references for rational use of antibiotics.METHODS Totally 6000 prescriptions from Jan.2013 to Dec.2013 were selected randomly.The proportion,types,combination and administration routes of antimicrobial agents were analyzed.SPSS 17.0was adopted for statistical analysis.RESULTS The antimicrobial agents applied in 1867 prescriptions,accounted for31.12%.There were 206 unreasonable prescriptions,including no indication,irrational selection,improper usage and dosage and others.Those 1867 prescriptions included 1316 of single usage,406 of duplex medication and 145 of triple and above combinations,accounted for 70.49%,21.75% and 7.76%,respectively.CONCLUSION The clinical application of antibacterial drugs in our hospital had several problems,such as no indication,a high proportion,improper combination medication,unreasonable administration route and poor drug selection.We should continue to strengthen the management of the use of antimicrobial agents.
