主动脉术后脑积液引流对脊髓保护作用的临床意义

摘　　要：【摘要】目的探讨主动脉术后脑脊液引流（CSFD）对脊髓的保护作用。方法主动脉手术患者34例，按术后是否行CSFD随机分为CSFD组18例和对照组16例，统计术后神经系统并发症发生率及转归。结果对照组出现4例神经系统（CNS）并发症：下肢截瘫合并多脏器功能不全、ARDS死亡1例、下肢轻瘫3例。CSFD组出现2例CNS并发症：下肢轻瘫合并脑梗死1例，放弃治疗，另1例下肢轻瘫患者出现血性脑脊液，伴呕吐、头痛，夹闭引流管后好转。结论脑脊液选择性引流对脊髓有明确的保护作用，可以作为常规的主动脉术中脊髓保护性措施。Objective To investigate the protective effect of cerebrospinal fluid drainage on spinal cord after major vascular surgery. Methods After the operation, 34 patients were randomly divided into CSFD group and control group, 18 cases in group CSFD and 16 cases in control group, incidence and prognosis of postoperative neurological complications were recorded. Results In the control group, there were 4 cases of nervous system complications, lower limb paraplegia combined with multiple organ dysfunction, ARDS death in 1 case, 3 cases of lower paralysis. CSFD group of 2 cases of neurological complications, lower limb paralysis with cerebral infarction 1 case who later gave up treatment, the other 1 case of lower limb paralysis patients with hemorrhagic brain effusion, with vomiting, headache, which was imporved after clamping the drainage tube. Conclusion Selective drainage of cerebrospinal fluid may have a protective effect on the spinal cord, which can be used as a protective measure of spinal cord in conventional large vascular surgery.
