知柏地黄汤对阴虚火旺型2型糖尿病患者胰岛素抵抗指数的影响

摘　　要：目的探讨知柏地黄汤对阴虚火旺型2型糖尿病患者稳态模型的胰岛素抵抗指数(HOMA-IR)的影响,进而评估对胰岛素抵抗的影响。方法选取2型糖尿病属阴虚火旺型患者100例,采用前瞻性随机单盲对照试验方法,其中对照组50例给予二甲双胍片口服;治疗组50例,给予知柏地黄汤联合二甲双胍片口服,隔日1剂,2组观察周期为2个月。测定2组治疗治疗前后的空腹血糖(FPG)和空腹胰岛素(FIns),按照公式Homa-IR=FIns×FPG/22.5(其中FIns单位为μU/m L,FPG单位为mmol/L)计算出Homa-IR。结果 2组均可不同程度降低HOMA-IR水平。经统计学处理,治疗组降低HOMA-IR程度大于对照组,即在改善胰岛素抵抗方面基础治疗+二甲双胍片+知柏地黄汤优于基础治疗+二甲双胍片。结论知柏地黄汤能有效改善阴虚火旺型2型糖尿病患者的胰岛素抵抗。Objective Effect of Zhibai Dihuang decoction of type of hyperactivity of fire due to yin deficiency and hyperactivity in type 2 diabetic patients with homeostasis model insulin resistance index（HOMA-IR） and evaluate the effect on insulin resistance. Methods Selected type 2 diabetes belongs to Yin-deficiency and Fire-excess,100 cases of patients with type,using prospective,randomized,single blind control test method. In the control group（50 cases） was given metformin tablets;treatment group（50 cases）,treated with Zhibai Dihuang decoction,every other day 1 agent,combined with metformin hydrochloride tablets and cycle in the two groups for 2 months. Fasting blood glucose（FPG） and fasting insulin（FIns） were measured before and after treatment in two groups,according to the formula Homa-IR=FIns＊FPG/22. 5（which is FIns units of U/ml,FPG units for mmol/L）to calculate the Homa-IR. Results The two groups can reduce the level of HOMA-IR in different degree. The statistical data indicated that the treatment group decreased HOMA-IR than in the control group,that is,in the aspects basic treatment plus metformin ＋Zhibai Dihuang decoction are better than basic treatment ＋metformin tablets of improving insulin resistance. Conclusion Zhibai Dihuang decoction can effectively improve Yin-deficiency and Fire-excess type 2 diabetic patients with insulin resistance.
