开放植骨结合负压封闭引流治疗感染性骨不连的临床研究

摘　　要：目的通过对比分析使用开放植骨结合负压封闭引流与传统方法治疗感染性骨不连的疗效,探讨感染性骨不连的治疗方法。方法回顾性分析2009年3月~2014年5月本院收治的43例胫骨感染性骨不连患者的临床及随访资料,按不同的治疗方法,分为开放植骨结合负压封闭引流组（22例）、传统方法治疗组（21例）,总结两组患者伤口愈合时间、骨折愈合时间、住院治疗费用、手术次数、功能恢复等情况,进行比较分析。结果开放植骨结合负压封闭引流组的伤口愈合时间及骨折愈合时间比传统方法治疗组短,而住院治疗费用、手术次数比传统方法治疗组少,两组比较差异有统计学意义（P〈0.05）。负压封闭引流组功能恢复优良率为72.72%,传统方法治疗组优良率为38.10%,两组比较差异有统计学意义（P〈0.05）。结论开放植骨结合负压封闭引流方法在治疗感染性骨不连方面有较好的疗效,能使患者获得更好的功能恢复,值得临床应用。Objective To explore treatment methods by comparing and analyzing curative effects of open bone grafting combined with vacuum sealing drainage or conventional methods in treatment of infected nonunion. Methods Clinical and follow-up data of 43 patients with tibial infected nonunion who were admitted to our hospital were retrospectively analyzed. They were allocated to the open bone grafting combined with vacuum sealing drainage group（22 cases） and the conventional treatment group（24 cases） according to treatment methods.Wound healing time, fracture healing time, hospitalization expenses, surgery times,functional recovery and so on of patients in two groups were compared. Results Wound healing time and fracture healing time of the open bone grafting combined with vacuum sealing drainage group were both shorter than those of the conventional treatment group. But hospitalization expenses and surgery times were less than those of the conventional treatment group.Comparative differences of two groups had statistical significance（P〈 0.05）. Excellent rates of the open bone grafting combined with vacuum sealing drainage group and the conventional treatment group were 72.72% and 38.10% respectively. Comparative differences of two groups had statistical significance（P〈 0.05）. Conclusion Open bone grafting combined with vacuum sealing drainage in treatment of infected nonunion has better curative effect. In addition,it can help patients to achieve better function recovery, worthy of clinical application.The method of open bone graft combined with closed negative pressure drainage in the treatment of infective bone nonunion is effective. It can make the patients get better functional recovery. The method is worthy to clinical application.
