摘 要:目的探讨PDCA循环在医院药剂科管理中的应用,分析提高药品管理质量、实现合理用药实践做法。方法运用PDCA循环理论进行数据分析,找出问题原因,并制定相应对策进行干预。将2013年2月~2014年1月采用PDCA循环后所发放的65782件药品设为观察组,将2012年1月~2013年1月采用PDCA循环前所发放的54780件药品设为对照组。观察采用PDCA循环法前后药库药品管理情况;另抽取我院1000份病历,其中采用PDCA循环前500份病历做为对照组,分析临床药物使用的医嘱共计21641项;采用PDCA循环法后的500份病历为观察组,分析临床药物使用的医嘱共计19859项。观察实施PDCA循环法前后药品临床使用情况。结果实施PDCA循环法管理后药物不合理医嘱率明显低于实施PDCA循环法管理前,差异有统计学意义(P〈0.05);调查发现采用PDCA循环前所发放的54780件药品中,药品有效使用率为96.90%,药品去向明确率为98.45%,药品不合格报损率为3.10%。采用PDCA循环后所发放的65782件药品中,药品有效使用率为98.80%,药品去向明确率为99.45%,药品不合格报损率为1.10%。实施PDCA循环法管理后药品有效使用率、药品去向明确率明显提高,与对照组比较差异有统计学意义(P〈0.05);而实施PDCA循环法管理后药品不合格报损率明显低于对照组,差异有统计学意义(P〈0.05)。结论医院药剂科管理使用PDCA循环能帮助规范医院药品管理流程,加强医疗药品管理质量,改善不合理用药情况。Objective To explore the application of Plan-Do-Check-Action(PDCA) cycle in the management of department of pharmacy in hospital to analyze the practice to improve quality of medicine management and achieve rational drug use. Methods PDCA theory was used to analyze the data, find out the causes of problem and formulate the corresponding countermeasures to intervene. 65782 drugs given out after adoption of PDCA cycle from February 2013 to January 2014 were set as the observation group. 54780 drugs given out before adoption of PDCA cycle from January 2012 to January 2013 were set as the control group. Drug managements of drug storehouse before and after adoption of PDCA cycle were observed. In addition, 1000 medical records were selected. Of which, 500 medical records before adoption of PDCA cycle were set as the control group and 21641 pieces of doctor's advises on clinical drug use were analyzed. While 500 medical records after the adoption of PDCA cycle were set as the control group and 19859 pieces of doctor's advises on clinical drug use were analyzed. Clinical uses of drugs before and after adoption of PDCA cycle were compared. Results Unreasonable drug orders after implementing method of PDCA cycle management rate was significantly lower than before the implementation method of PDCA cycle management, and the difference had statistical significance(P〈 0.05). In 54780 investigated drugs, rate of effective utilization of drugs was 96.90%, rate of accurate whereabouts of drugs was 98.45% and loss rate of unqualified drugs was 3.10%. After implementation of PDCA cycle, in 65782 investigated drugs, rate of effective utilization of drugs was 98.80%, rate of accurate whereabouts of drugs was 99.45% and loss rate of unqualified drugs was 1.10%. After implementation of PDCA cycle, rate of effective utilization and rate of accurate whereabouts of drugs were significantly improved. And compared with those before implementation of PDCA cycle, the difference had statistical significance(P〈 0.05). Loss
