经鼻内窥镜下鼻中隔穿孔修补术治疗鼻中隔穿孔的临床效果观察

摘　　要：目的分析经鼻内窥镜下鼻中隔穿孔修补术治疗鼻中隔穿孔的临床效果。方法选取2011年2月~2013年2月收治于我院的40例鼻中隔穿孔患者作为研究对象,并根据术式不同分为观察组和对照组,观察组20例,行双旋转鼻中隔黏膜瓣修补术;对照组20例,行颞肌、骼前上棘阔筋膜修补术。两组手术均经鼻内窥镜下进行,对比两种术式的修复效果。结果观察组患者无需拆线,所有患者呈Ⅰ期愈合,鼻腔通气全部恢复到正常水平,且无再穿孔现象出现。对照组患者需拆线,术后2年有4例患者发生阔筋膜干燥脱落而引发再穿孔,14例患者鼻腔通气恢复到正常水平。结论鼻中隔穿孔患者的修补术式中,双旋转鼻中隔黏膜瓣修补鼻中隔穿孔,其治愈率高、生长效果良好。Objective To explore the clinical effect of nasal septum perforation repair under nasal endoscopy in the treatment of nasal septum perforation. Methods 40 patients with nasal septum perforation who were in our hospital from February 2011 to February 2013 were divided into observation group and control group according to surgical methods. There were 20 cases in the observation group, who received double rotation nasal septum mucosal flap repair, and there were another 20 in the control group, who received temporal muscle and anterior superior iliac spine fascia lata repair. Operations of two groups were both performed under nasal endoscope, and the effects of two surgical methods were compared. Results In the observation group, all patients didn＇t need to remove suture and acquired Ⅰ stage healing. Their nasal ventilation returned to normal levels, and there was no re-perforation phenomenon. In the control group, the patients needed to remove the suture. There were 4 cases suffered re-perforation caused by the drying and detaching of fascia lata within 2 years after surgery, 14 cases had their nasal ventilation returned to normal levels. Conclusion Among surgical methods of nasal septum perforation repair, the double rotation nasal septum mucosa flap repair has much higher cure rate and much better growth effect.
