摘　　要：目的 探讨腓骨长肌腱移植对踝关节和足功能的影响.方法 入选2009年1月至2013年6月在河北省唐山市人民医院治疗的前、后交叉韧带损伤患者30例,在第5趾骨基底近段1.5 cm处切取腓骨长肌腱用于重建前后交叉韧带,保留的1.5 cm与腓骨短肌腱缝合.术前和术后12、24个月分别进行踝关节功能评测（Kofoed评分）和足功能评测（Maryland评分）,并根据评分将踝关节功能和足功能分为优、良、可、差4个等级.结果 经随机区组设计的方差分析,30例患者手术前和手术后12、24个月的Kofoed评分[（85.2±11.0）分、（86.5±12.6）分、（87.5±12.0）分]和Maryland评分[（88±7）分、（90±8）分、（90±8）分]差异均无统计学意义（均P＞0.05）;经Ridit检验,手术前和手术后12、24个月的踝关节Kofoed等级平均秩次分别为0.496、0.511、0.492,足Maryland等级平均秩次分别为0.504、0.505、0.491,差异均无统计学意义（均P ＞0.05）;手术前和手术后12、24个月,踝关节功能优良率[90.0％（27/30）、90.0％（27/30）、93.3％ （28/30）]及足功能优良率[90.0％（27/30）、86.7％ （26/30）、86.7％（26/30）]差异均无统计学意义（均P＞0.05）.结论 腓骨长肌腱移植对踝关节和足功能无明显影响.Objective To investigate the effects of peroneus longus tendon grafting on ankle and foot function.Methods Thirty patients with anterior and posterior cruciate ligament injury were enrolled from January 2009 to June 2013 and underwent peroneus longus tendon grafting.The peroneus longus tendon 1.5 cm near the fifth phalanx base was cut for reconstruction of anterior and posterior cruciate ligament,the retain 1.5 cm peroneus longus tendon was sutured with peroneus brevis tendon.The ankle function evaluation （Kofoed score） and foot function evaluation （Maryland score） before operation,12 and 24 months after operation were evaluated and divided into four grade：excellent,good,common and bad.Results The analysis of variance for randomized completeblock design showed that the Kofoed score and Maryland score were not significantly different among before operation,12 and 24 months after operation [（85.2 ± 11.0） scores,（86.5 ± 12.6） scores,（87.5 ± 12.0） scores;（88±7） scores,（90 ± 8）scores,（90 ± 8）scores];the Ridit test showed that the mean ranks of Kofoed grade and Maryland grade before operation,12 and 24 months after operation were 0.496,0.511,0.492 and 0.504,0.505,0.491,with no significantly changes （P 〉 0.05）;x2 test showed that the excellent rates of Kofoed score and Maryland score before operation,12 and 24 months after operation were 90.0% （27/30）,90.0% （27/30）,93.3% （28/30） and 90.0% （27/30）,86.7% （26/30）,86.7% （26/30）,no significantly changes （P 〉 0.05）.Conclusion Peroneus longus tendon grafting has no significant influence on the ankle and foot functions.
