摘 要:目的 探讨心脏磁共振成像特征性追踪技术(FT-CMR)评价健康人左心房应变及应变率的可行性.方法 选择2015年1-6月首都医科大学附属北京安贞医院行心脏磁共振成像(CMR)检查的健康体检者30名作为研究对象,CMR检查扫描包括真实稳态自由进动(SSFP)序列.将所有受试者按年龄分为20~34岁组、35~49岁组、50~72岁组,各10名.应用CVI和QMass离线心血管分析软件分别检测FT-CMR参数:左心房应变、应变率参数和容积功能参数.左心房应变、应变率参数包括总应变(Es)、被动应变(Ee)、主动应变(Ea)、正向应变率峰值(SRs)、心室舒张早期负向应变率峰值(SRe)、心室舒张晚期负向应变率峰值(SRa),容积功能参数包括左心房最大容积(LAVmax)、左心房最小容积(LAVmin)、左心房收缩前容积(LAVp)、左心房总射血分数(LATEF)、左心房被动射血分数(LAPEF)、左心房主动射血分数(LAAEF).结果 30名健康人左心房Es为(36±11)%,Ee为(21±10)%,Ea为(15±4)%,SRs为(214±71)%/s,SRe为(-264±111)%/s,SRa为(-231±62)%/s,LAVmax为(37±9) ml/m^2,LAVmin为(16±4) ml/m^2,LAVp为(24±5)ml/m^2,LATEF为(55±8)%,LAPEF为(33±8)%,LAAEF为(22±5)%.比较各年龄组的应变及应变率指标,Es、Ee、SRs、SRe在不同年龄组之间差异有统计学意义(P<0.05).左心房Es、Ee、SRs、SRe的绝对值均随年龄的增大而减小.健康体检者Es、SRs分别与LATEF存在良好的相关性;Ee、SRe分别与LAPEF存在良好的相关性;Ea、SRa分别与LAAEF存在良好的相关性(均P<0.05).所有受试者左心房应变及应变率参数观察者内、观察者间的组内相关系数和Bland-Altman曲线均显示出良好的一致性.结论 FT-CMR评价左心房应变及应变率是可行的,具有良好的临床应用价值.Objective To evaluate the feasibility of left atrial strain and strain rate with feature tracking cardiac magnetic resonance imaging(FT-CMR) in healthy people.Methods Thirty healthy subjects were enrolled from January to June 2015 and divided into 3 groups according to the age (20 to 34 years,35 to 49 years,50 to 72 years),each group of 10 people.All people had cardiac magnetic resonance imaging (CMR) and scanning included real steady-state free precession(SSFP) sequence.Those were measured FT-CMR parameters(left atrial strain and strain rate parameters,left atrial volume and function parameters) by using CVI and QMass offline cardiovascular analysis software respectively.Left atrial strain and strain rate parameters included left atrial total strain(Es),passive strain(Ee),active strain(Ea),peak positive strain rate (SRs),peak early negative strain rate(SRe) and peak late negative strain rate(SRa).Left atrial volume and function parameters included maximum left atrial volume (LAVmax),minimum left atrial volume (LAVmin),Pre-atrial contraction left atrial volume (LAVpre-ac),Total left atrial emptying fraction (LATEF),passive left atrial emptying fraction (LAPEF) and active left atrial emptying fraction(LAAEF).Results Left atrial strain,strain rate,left atrial volume and function parameters in thirty healthy subjects were as follows:Es:(36 ± 11) %,Ee:(21 ± 10) %,Ea:(15 ± 4) %,SRs:(214±71)%/s,SRe:(-264 ± lll)%/s,SRa:(-231 ±62)%/s;LAVmax:(37 ±9)ml/m2,LAVmin:(16 ± 4) ml/m2,LAVpre-ac:(24 ± 5) ml/m2,LATEF:(55 ± 8) %,LAPEF:(33 ± 8) %,LAAEF:(22 ± 5) %.Compared with strain and strain rate in various age groups,Es,Ee,SRs and SRe between different age groups was significant difference respectively(P 〈 0.05).The absolute value of left atrial Es,Ee,SRs and SRe decreased with the increasing age.There were good correlations between Es,SRs with LATEF;Ee,SRe with LAPEF;Ea,SR
