Pilon胫骨骨折应用两种内固定法治疗的效果及其安全性比较

摘　　要：目的比较旋入式自锁钉和加压钢板内固定法治疗Pilon胫骨骨折的效果及其安全性进行,为其临床治疗提供可靠依据。方法选择2013年2月至2015年8月期间在医院接受治疗的96例Ⅲ型Pilon胫骨骨折患者,将其随机分为观察组和对照组各48例。对照组实施加压钢板内固定手术治疗,观察组则实施旋入式自锁钉内固定手术治疗。观察、记录两组患者的手术情况、膝关节功能恢复情况(使用HSS膝关节功能评分以及Rasmussen膝关节功能评分进行评价)以及并发症情况等。结果在手术时间、住院时间、术中出血量、下床活动时间以及骨折愈合时间等方面,观察组均明显的优于对照组,差异具有统计学意义(P<0.05);观察组膝关节恢复情况均明显好于对照组,差异具有统计学意义(P<0.05);观察组并发症情况明显少于对照组,差异具有统计学意义(P<0.05)。结论与加压钢板内固定术相比较,旋入式自锁钉内固定术治疗Pilon胫骨骨折,能够获得更好的临床效果,有效促进患者的健康恢复,并且具有较少的并发症,值得推广。Objective To research the different kinds of internal fixations on effects and safety of patients with Pilon tibial fracture,to provide a reliable basis for clinical treatment. Methods 96 cases of patients with type Ⅲ Pilon tibial fractures were selected,which were treated in our hospital from February 2013 to August 2015,and were divided into the observation group（ 48 cases） and control group（ 48 cases）. The patients of observation group were treated with screw self- locking screw fixation surgery,and the patients of control group were treated with compression plate fixation. To observe and compare the surgery,the knee function recovery（ evaluated by the HSS knee function score and Rasmussen knee function score） and complications of two groups. Results The operative time,hospital stay,blood loss,ambulation and fracture healing time of the observation group was significantly better than those of the control group（ P〈0. 05）. The knee recovery of the observation group was significantly better than those of the control group（ P〈0. 05）. The complications of the observation group was significantly less than those of the control group（ P〈0. 05）. Conclusion With treating Pilon tibial fractures,screw self- locking screw fixation can obtain satisfactory clinical results and effectively promote the healthy recovery of patients,and it is also has fewer complications,which makes it worthy of promotion.
