妊娠期高血压疾病患者血清尿素氮、肌酐和尿酸水平变化的探讨

摘　　要：目的探讨妊娠期高血压疾病患者血清尿氮素（UN）、肌酐（Cr）、尿酸（UA）变化及意义。方法选取194例妊娠期高血压疾病患者为研究对象,其中妊娠期高血压者70例（A组）、轻度子痫前期者64例（B组）、重度子痫前期者60例（C组）。选取80例健康妊娠妇女为对照组。比较各组血清UN、Cr及UA表达水平。结果对照组、A组、B组和C组患者UN、Cr水平比较无统计学意义（P〉0.05）;A组、B组和C组患者UA水平明显高于对照组,组间比较差异显著（P357.0μmol/L的妊娠期高血压疾病患者,其胎儿生长受限、胎儿窘迫、新生儿窒息和围产儿死亡发生率明显高于血清尿酸水平〈297.5μmol/L者和血清尿酸水平297.5-357.0μmol/L者（P〈0.05）。结论血清UA水平与妊娠期高血压疾病病情严重程度和围生儿预后密切相关。Objective To evaluate the changes and significance of serum urea nitrogen（ UN）,creatinine（ Cr） and uric acid（ UA） in pregnant women with hypertensive disorder complicating pregnancy. Methods 194 pregnant women with hypertensive disorder complicating pregnancy were selected as the research object,70 cases for gestational hypertension（ A group）,64 cases for mild preeclampsia（ B group）,60 cases for severe preeclampsia（ C group）. 80 healthy pregnant women were chosen for control group. Results The level of UN and Cr in control group,A group,B group and C group were no significant differences（ P〈0. 05）. The UA level of A group,B group and C group were significantlyhigher than control group,showed significant differences between the each groups（ P〈0. 05）. The UA level of B group and C group were significantly higher than that of group（ P〈0. 05）. The UA level of C group were significantly higher than that of B group（ P〈0. 05）. The rate of fetal growth restriction,fetal distress,neonatal asphyxia and perinatal death of pregnant women with hypertensive disorder complicating pregnancy,who was serum UA 357. 0 μmol / L,were significantly higher than the cases with serum UA 297. 5 μmol / L and serum UA 297. 5 ~ 357. 0μmol / L（ P〈0. 05）. Conclusion Serum UA level with hypertensive disorder complicating pregnancy and perinatal prognosis is closely related.
