局部病灶清理+灌洗治疗脊柱结核的疗效观察

摘　　要：目的观察微创下局部病灶清理＋灌洗治疗脊柱结核的疗效。方法自2010年11月-2014年6月在椎间盘镜或可扩张通道辅助下,经椎间孔外或椎弓根或椎板开窗入路局部病灶清理＋置管灌洗治疗脊柱结核22例,所有患者均在全身结核药化疗的基础上进行。结果全组病例手术顺利,21例随访14月-5年痊愈,1例多发腰椎结核患者术后12个月复发,二次放手术后愈合,局部灌洗7-21 d,平均（16.0±3.2）d。结论在椎间盘镜或可扩张通道辅助下,经椎间孔外或椎弓根或椎板开窗入路局部病灶清理＋置管灌洗治疗脊柱结核疗效确切,创伤小,费用较低。Objective To evaluate the clinical efficacy and surgical treatment of spinal tuberculosis treated by minimally invasive operation. Methods since Nov 2010 to Jun 2014,22 patients with spinal tuberculosis patients were operated under miccroendoscopic or quardrent system. Results All these patients were operated successfully,21 patients were followed up with an average of 2.3 years,all these patients were recoveried,1 patients recurred,partial lavation were treated since 7 to 21 days average of（16.0±3.2）d. Conclusion Select appropriate approach according to the position of the spinal tuberculosis,operated under miccroendoscopic or quardrent system,limited bridement,peritoneal lavage and drainage is definite effect,minimal invasive,economical.
