摘 要:目的探讨磁共振弥散加权成像(DWI)在骨肉瘤诊断及鉴别诊断中的价值。方法选择临床资料完整的33例骨肉瘤患者,先行常规MRI平扫,然后依次进行不同b值的DWI扫描。结果 (1)33例骨肉瘤均表现为肿瘤边缘形态不规则,边界不清,病变内信号不均匀和病灶周围有骨髓水肿;(2)33例骨肉瘤DWI上均表现为高信号;(3)b值取400、800、1 000和1 500 s/mm2时,骨肉瘤实质区ADC值大于邻近正常骨髓的ADC值;而且病变实质区ADC值和邻近正常骨髓ADC值与b值的大小呈负相关,即ADC值随着b值的增大而降低。结论磁共振平扫和扩散加权成像有助于骨肉瘤的诊断。Objective This study is to analyze osteosarcoma in diffusion weighted imaging(DWI)to explore the clinical diagnosis and differential diagnosis of them. Methods 33 cases were selected,the first step was MRI plain scan,and then DCE-MRI by successively setting many different b values. Results(1)33 patients with osteosarcoma showed the tumor edge irregular,ill-defined,heterogeneous signal intensity lesions and lesions surrounding bone marrow edema.(2)33 patients with osteosarcoma showed high signal on DWI.(3)When taking b values were 400、800、1 000 and 1 500 s/mm2,the ADC values of parenchyma lesion of the osteosarcoma were much larger than the those of normal bone marrow,moreover the ADC value and b value was a negative correlation,which was between parenchyma of the osteosarcoma lesions and adjacent normal the bone marrow,that was,as the b value increased,ADC value decreased. Conclusion Plain MRI and DWI might be helpful for the diagnosis osteosarcoma.
