中国汉族特应性皮炎4757例患者过敏史相关性分析

摘　　要：目的探讨中国汉族特应性皮炎(AD)患者同时患有过敏性鼻炎(allergic rhinitis,AR)、哮喘(asthma,AS)等疾病时对AD表型的影响。方法利用2001年11月-2010年5月收集的4 757例AD患者资料,采用SPSS20.0软件进行卡方检验,分析不同过敏史对AD表型的影响。结果 1AD同时患有AR与南方地区(P=1.51×10^(-18))正相关,与伴发耳廓下裂隙(P=1.75×10^(-7))负相关,与其他表型无关;2AD同时患有AS与南方地区(P=4.39×10^(-20))、掌纹症(P=5.06×10^(-9))、眶周黑晕(P=3.10×10^(-7))正相关,与伴发干皮症(P=2.13×10^(-6))、早年发病(P=1.35×10^(-3))、耳廓下裂隙(P=1.35×10^(-4))负相关,与其他表型无关;3AD同时患有AR+AS与南方地区(P=2.76×10^(-13))、伴发掌纹症(P=1.70×10^(-5))、眶周黑晕(P=2.42×10^(-5))正相关,与其他表型无关;4AD伴过敏家族史与毛周角化症(P=1.46×10^(-3))、耳廓下裂隙(P=3.34×10^(-5))正相关,与南方地区(P=4.63×10^(-32))负相关,与其他表型无关。结论 AD伴发不同过敏史与不同地域有关,同时影响伴发症状的发生。Objective To investigate the associations between allergic rhinitis （ AR）, asthma （AS） or other allergic history and phenotype of atopic dermatitis （AD） in the Chinese Han population. Methods Data from 4 757 AD patients collected from November 2001 to May 2010 were used in the study. SPSS 20. 0 software was used for chi-square test to determine the associations between allergic history and phenotype of AD. Results AD accompanied with AR was positively correlated with southern region （P = 1.51 ×10^-18） ,negatively associated with auricular fissure （ P = 1.75×10^-7 ）, and independent of other phenotypes. AD accompanied with AS was positively correlated with southern region （ P = 4. 39 ×10^-20 ）, palmar hyperlinearity （ P = 5.06 ×10^-9） and orbital darkening （P = 3.10 ×10^-7） ,but negatively correlated with xeroderma （P = 2. 13×10^-6） ,early onset （P = 1.35 ×10^-3 ）and auricular fissure （P = 1.35×10^-4） ,and independent of other phenotypes. AD accompanied with AR ＋ AS was positively associated with southern region （ P = 2. 76 ×10^-13 ）, palmar hyperlinearity （P = 1.70 ×10^-5 ） and orbital darkening （ P = 2. 42 ×10^-5 ）, and independent of other phenotypes. AD accompanied with family atopic history was positively correlated with ichthyosis （P = 1.46 ×10^-3） and keratosis pilaris （P = 3.34×10^-5 ） , negatively correlated with southern region （P = 4. 63 ×10^-32 ） , and independent of other phenotypes. Conclusion Allergic history is associated with multiple factors of AD,including region, and some secondary characteristics.
