摘 要:目的 观察大鼠单侧输尿管梗阻(UUO) 24 h内肾脏水通道蛋白2(AQP2)表达变化及其对肾脏功能的影响.方法 28只慕尼黑大鼠行单侧输尿管结扎术,分别于术后2h(UUO-2组)、6h(UUO-6组)、12h(UUO-12组)、24h(UUO-24组)收集血液和肾脏标本(n=7),检测血浆电解质和渗透压,Western blot检测肾脏AQP2蛋白水平.各组均设等量(n=7)假手术对照组(Sham-2、Sham-6、Sham-12、Sham-24组).结果 与Sham组比较,UUO-24 h血浆渗透压升高[(347.6±1.7)比(304.3±1.3) mosmol/kg H2O],UUO-2 h血浆肌酐[(39.6±5.8)比(10.5±4.3)μmol/L]、尿素[(9.2±0.4)比(6.9±0.3) mmol/L]和血钾[(5.3±0.2)比(4.3 ±0.1)mmol/L]升高,UUO-2、UUO-6、UUO-12组,24h血浆肌酐呈上升趋势.UUO-6、UUO-12组,24h血钠水平显著低于Sham组[(135.6±1.3)比(137.4±0.8),(126.3±1.3)比(136.5±1.2),(135.2±1.0)比(139.7±1.5) mmol/L].Western blot显示UUO-12 h和UUO-24 h导致AQP2在肾脏内髓表达分别下降到Sham组的69%和22%,各组比较差异有统计学意义(P<0.05).结论 单侧输尿管梗阻后12h开始出现肾脏AQP2蛋白表达显著下降,肾脏功能改变早于AQP2蛋白变化.Objective To investigate the early time course of dysregulation of aquaporin 2 (AQP2) and renal function after 2-,6-,12-,24-h unilateral ureteral obstruction (UUO).Methods Twenty-eight Munich-Wistar rats were subjected to UUO,and at 2 h (UUO-2,n =7),6 h (UUO-6,n =7),12 h (UUO-12,n =7),and 24 h (UUO-24,n =7),blood and kidney samples were collected.Age-and time-matched sham-operated controls (n =7) were prepared and observed in parallel for each UUO group.Between 2 and 24 h,osmolality and electrolyte concentrations in plasma were determined.The dysregulation of AQP2 was detected by by semiquantitative immunoblottling.Results As compared with sham group,2-h UUO resulted in a significant increases in the level of plasma creatinine [(39.6±5.8) vs.(10.5±4.3) μmol/L],urea [(9.2±0.4) vs.(6.9±0.3) mmol/L],and serum potassium [(5.3 ±0.2) vs.(4.3 ±0.1) mmol/L].Rats subjected to 6-,12-and 24-h UUO had decreased plasma sodium levels [(135.6 ± 1.3) vs.(137.4 ± 0.8) mmol/L,(126.3 ± 1.3) vs.(136.5 ±1.2) mmol/L,and (135.2 ± 1.0) vs.(139.7 ± 1.5) mmol/L,respectively] as compared with sham group.Plasma osmolality was higher after 24-h UUO than in sham group [(347.6 ± 1.7) vs.(304.3 ± 1.3) mosmoL/kg H2O].The AQP2 protein abundance did not change in the kidneys from 2-and 6-h UUO rats,but after 12-and 24-h UUO the significant down-regulation was observed (69% and 22%,respectively) as compared with sham group.Conclusion AQP2 protein abundance did not change in obstructed kidneys from 2-and 6-h UUO rats,but is significantly down-regulated after 12-and 24-h UUO.The damage of renal function observed is earlier than the down-regulation of AQP2 protein.
