摘 要:目的 探讨食管癌上皮组织中CD57^+自然杀伤细胞(NK)及CD68^+巨噬细胞浸润密度与临床病理特征的相关性及对食管癌患者预后的影响.方法 应用免疫组织化学方法检测138例Ⅱ~Ⅲ期食管癌患者组织中CD57^+ NK及CD68^+巨噬细胞的浸润水平,采用x2检验分析其浸润密度与临床病理特征的相关性,拟合多因素Cox模型分析上述指标对食管癌患者生存时间及预后的影响.结果 CD57^+ NK及CD68^+巨噬细胞浸润水平与人口学及临床病理特征无明显相关;在调整了性别、年龄、肿瘤大小及TNM分期的影响后,食管癌肿瘤组织中CD57^+ NK细胞浸润水平高的患者具有更低的死亡风险[风险比(HR)=0.615,95%可信区间(CI):0.405~0.933,P<0.05];CD68^+巨噬细胞浸润水平高的食管癌患者具有更高的死亡风险(HR=1.499,95%CI:1.008~2.230,P <0.05);CD57^+ NK细胞浸润水平低并且CD68^+巨噬细胞浸润水平高的食管癌患者具有更高的死亡风险(HR=2.573,95% CI:1.408 ~4.701,P<0.01);联合CD57、CD68阳性细胞浸润密度构建的Cox模型具有更好的拟合优度.结论 食管癌组织中CD57^+ NK和CD68^+巨噬细胞浸润密度均为独立预后因素,两者联合检测具有更优的预后判断能力.Objective To evaluate the prognostic role of CD57^+ natural killer (NK) cells and CD68^+ macrophages infiltration in esophagus cancer tissue.Methods Immunohistochemical method was constructed to detect the infiltration density of CD57^+ NK cells and CD68^+ macrophages in 138 cases of esophagus cancer tissue.Chi-square test was used to analyze the correlation between the infiltration density of these two cells and the clinicopathological parameters.The association between CD57^+,CD68^+ cells and hazard ratio was assessed with overall survival (OS) in multivariate Cox models.Results There was no significant correlation between infiltration density of CD57^+ NK cells,CD68^+ macrophages and clinical parameters.When "Gender","Age","Tumor size" and "TNM stage" were adjusted,high tumor-infiltrating density of CD57^+ NK cells independently predicted better outcomes [hazard ratio (HR) =0.615,95% confidence interval (CI):0.405-0.933,P 〈0.05],but high tumor-infiltrating density of CD68^+ macrophages independently predicted worse outcomes (HR =1.499,95 % CI:1.008-2.230,P 〈 0.05).Patients with low tumor-infiltrating density of CD57^+ NK cells and high density of CD68^+ macrophages associated with a worse outcomes than patients with high CD57^+ cells and low CD68^+ cells infiltrating (HR =2.573,95% CI:1.408-4.701,P 〈0.01),and this multivariate Cox model was fitted better than other two models above-metioned.Conclusion The tumor-infiltrating density of CD57^+ NK cells,and CD68^+ macrophages independently predict the survival outcomes in patients with stage Ⅱ-Ⅲ esophagus cancer.Combination of tumor-infiltrating CD57^+ NK cells and CD68^+ macrophages as a factor had a more powerful effect in prodicting prognosis.
