尿道板纵切卷管尿道成形术和包皮内板岛状皮瓣加盖尿道成形术治疗尿道下裂的临床疗效

摘　　要：目的：探讨尿道板纵切卷管尿道成形术（tubularized incised plate,TIP）和包皮内板岛状皮瓣加盖尿道成形术（onlay island flap,OIF）治疗尿道下裂的临床疗效。方法：取2014年1月-2015年2月在我院治疗的尿道下裂患儿71例,其中接受TIP治疗的患儿40例（TIP组）,接受OIF治疗的患儿31例（OIF组）,观察两组患儿的手术时间、治疗效果、并发症发生情况等。结果：TIP组和OIF组患儿年龄及病例类型差异无统计学意义（P〉0.05）;TIP组手术时间为（82.13±7.94）min,明显短于OIF组的（119.07±11.08）min,差异有统计许学意义（P〈0.05）。TIP组和OIF组一次手术成功率分别为85.00%和87.10%,差异无统计学意义（P〉0.05）。TIP组术后发生尿瘘6例,阴茎头裂开1列,发生率为17.50%;OIF组术后发生尿瘘5例,阴茎头裂开1例,发生率为19.35%,两组术后并发症比较,差异无统计学意义（P〉0.05）。结论：尿道板纵切卷管尿道成形术和包皮内板岛状皮瓣加盖尿道成形术治疗尿道下裂均有较好效果;尿道板纵切卷管尿道成形术具有操作简便且手术时间短等优点。Objective：To compare clinical efficacy between tubularized incised plate（TIP）and onlay island flap（OIF）urethroplasty on hypospadias.Method：From January 2014 to February 2015 in our hospital,71 cases of hypospadias were treated.Forty cases received TIP treatment（TIP group）and 31 patients received OIF treatment（OIF group）.The operation time,the treatment effect and the complications were observed and compared between the two groups.Result：There was no significant difference in age and type of case between TIP group and OIF group（P〈0.05）.The operation time of TIP group was（82.13±7.94）min,which was significantly shorter than that of OIF group（119.07±11.08）min.The difference was statistically significant（P〉0.05）.The success rate of single-session of the TIP group and OIF group were 85.00% and 87.10% respectively,which the difference was not statistically significant（P〉0.05）.Postoperative urinary fistula was found in six cases of TIP group,and one case experienced glans split.The incidence rate was 17.50%.Postoperative urinary fistula was found in five cases of OIF group,and also only one case experienced glans split.The incidence rate was 19.35%.The difference of incidence rate of postoperative complications between two groups showed no statistical significance（P〉0.05）.Conclusion：Both TIP and OIF urethroplasty show good effects on hypospadias.However,TIP urethroplasty shows advantages of shorter operation time and easier to operate,etc.
