摘  要:目的:探讨经阴道三维彩色血管能量成像检测子宫内膜癌血流参数与肿瘤组织微血管密度(microvessel density,MVD)、Delta样配体4(Delta-like ligand 4,DLL4)、窖蛋白-1(Caveolin-1)表达的关系。材料与方法:对32例子宫内膜癌患者(内膜癌组)、28例子宫内膜不典型增生患者(内膜不典型增生组)和30例行全子宫切除术的患者(正常宫内膜组)均行经阴道三维彩色血管能量成像检测,采用免疫组织化学SP法检测内膜癌组组织中DLL4、Caveolin-1表达,分析子宫内膜癌超声血流动力学参数[阻力指数(resistance index,RI)、收缩期峰值流速(peak systolic velocity,PSV)、舒张期末流速(end diastolic velocity,EDV)、搏动指数(pulsatility index,PI)、血管指数(Vascularity index,VI)、血管血流指数(vascularization-flow index,VFI)、DLL4、Caveolin-1在子宫内膜癌组、子宫内膜不典型增生组织组、正常子宫内膜组织组的病例中病例中的表达差异及其相关性。结果:子宫内膜癌不同分期、不同病理类型的病例的超声血流动力学参数的阻力指数(the resistance index,RI)、收缩期峰值流速(peak systolic velocity,PSV)、舒张期末流速(end diastolic velocity,EDV)、搏动指数(pulsatility index,PI)、血管血流指数(vascularization-flow index,VFI)与MVD、DLL4表达相关,随着超声分期增高、浸润程度增加,超声血流分级增加、RI减小,病例组MVD值、DLL4表达高于对照组,差异有统计学意义(P<0.05);Caveolin-1在子宫内膜癌组织中未见表达。结论:联合检测子宫内膜癌患者的三维超声血流特征与MVD、DLL4对提高子宫内膜癌肌层浸润程度的判定、肿瘤的增殖能力、新生血管能力有重要的临床意义;Caveolin-1在子宫内膜癌组织中未见表达。Purpose: To investigate the relationship between the three dimensional energy flow characteristics of the endometrial carcinoma and the expression of microvessel density( MVD),delta like ligand 4( DLL 4) and Caveolin-1 in the tumor tissues. Materials and Methods: From February 2010 to December 2015,28 cases of patients with atypical hyperplasia of endometrium and 32 cases of patients with endometrial cancer were collected from the affiliated sanshui hospital of guangdong medical college. The expression of DLL4 gene and Caveolin-1 was detected by immunohistochemical staining in endometrial carcinoma to analyze the difference of ultrasonic parameters the expression of DLL4 and Caveolin-1 in different stages and different pathological types of endometrial carcinoma. Results: Ultrasound hemodynamic parameters of patients with different stages and different pathological types of endometrial carcinoma,such as resistance index( RI),systolic velocity peak( PSV),end diastolic velocity( EDV),pulsatility index( PI),vascularization-flow index( VFI) were related to MVD,DLL4 expression. With the increase of ultrasonic staging and the degree of infiltration,the ultrasound blood flow classification increased and RI decreased,and the expression of MVD value and DLL4 in the case group were significantly higher than those in the control group,the difference was statistically significant,p 0. 05. The expression of Caveolin-1 in endometrial carcinoma tissues was not found. Conclusion: Three dimensional ultrasound blood flow characteristics of endometrial carcinoma is closely related to MVD and DLL4. Combined detection has important clinical significance in the diagnosis of endometrial carcinoma.the degree of muscle invasion,tumor growth and angiogenesis. Caveolin-1 is not expressed in endometrial carcinoma.
