摘 要:目的探讨一种简化的全腹腔镜下胃造瘘术的临床应用价值。方法对12例行简化的全腹腔镜下胃造瘘术的临床资料进行回顾性分析,其中晚期食管癌8例,晚期贲门癌4例,均完全或几乎不能经口进食。结果 12例均顺利完成手术,手术时间20-55 min,平均25.1 min,无中转开腹者和手术死亡者,术后无胃穿孔、胃出血、造瘘管滑脱、吸入性肺炎等并发症,术后次日造瘘管均顺利开放并维持正常肠内营养。结论简化的全腹腔镜下胃造瘘术安全可行,创伤小,康复快,是长期胃肠内营养支持的有效途径,尤其适合因食管狭窄等原因无法行胃镜下胃造瘘术的患者。Objective To evaluate clinical application value of a simplified total laparoscopic gastrostomy. MethodsClinical data of 12 cases undergoing a simplified total laparoscopic gastrostomy was retrospectively analyzed, All of the 12 cases could hardly or almost could not take food orally, including 8 cases with advanced esophageal cancer and 4 cases of advanced cardiac carcinoma. Results The operation was successfully completed in all of the 12 cases without laparotomy. The operation time ranged from 20 to 55 minutes with an average of 25.1 minutes. No postoperative complications occurred. All fistulas were successfully open and normal enteral nutrition on the following day of surgery was maintained. Conclusion The simplified total laparoscopic gastrostomy is safe and effective with rapider postoperative recovery and smaller trauma. It is an effective method for long-term enteral nutrition, especially suitable for failed endoscopic gastrostomy in patients with esophagus stricture.
