摘 要:目的分析系统性红斑狼疮(SLE)相关血浆miRNA表达并评价其作为SLE活动的意义。方法收集27例高度活动SLE患者、21例SLE稳定期患者和10例健康对照者的血浆标本;利用miRNA芯片分析血浆miRNA差异表达;以cel-miR-39为外参,通过定量PCR验证候选miRNA表达,通过ROC曲线探讨血浆miRNA表达作为SLE活动临床评价的意义。结果 miRNA芯片检测显示,与健康对照者比较,SLE患者血浆存在明显差异表达的miRNA。定量PCR检测显示,与稳定SLE患者相比,miR-142-3p和miR-15b在SLE高度活动患者血浆中表达水平明显升高,miR-155*在SLE高度活动患者血浆中表达水平明显下调,差异均有统计学意义(P<0.05)。SLE患者血浆miR-142-3p、miR-155*区分SLE活动患者与稳定患者的ROC曲线下面积(AUC)分别为0.919、0.914。利用miR-142-3p与miR-155*联合评价的ROC曲线AUC为0.935。结论 SLE患者血浆存在明显差异表达的miRNA,其中上调表达的miR-142-3p及下调表达的miR-155*是SLE活动评价一个新的临床诊断生物学指标。Objective To analyze miRNA expression in the plasma of systemic lupus erythematosus( SLE) and to explore their clinical significance in assessing active SLE patients. Methods The plasma samples were collected from 27 cases of SLE patients with a high degree of activity,21 cases of stable SLE patients and 10 healthy volunteers. miRNA microarray was used to detect the plasma miRNA expression in the plasma. The expression of candidate miRNAs were identified by quantitative PCR,using cel- miR- 39 as the external reference. ROC curve was used to investigate the clinical value of these plasma candidate miRNAs in assessing active SLE patients. Results The miRNA microarray detection showed that compared with the healthy controls,plasma miRNA expression in SLE patients was significantly different. The data of quantitative PCR showed that miR-142- 3p and miR- 15 b expressions in the plasma from active SLE patients significantly increased,compared with the stable SLE patients,whereas miR- 155* level in the plasma from active SLE cases significantly decreased,with the difference statistically significant( P〈0. 05). Area under the curve( AUC) of plasma miR- 142- 3p and miR- 155* for distinguishing SLE patients with active or stable were 0. 919 and 0. 914,respectively. When combined with the detection of miR- 142- 3p and miR- 155*,AUC in ROC curve arrived 0. 935. Conclusion Significant differences exist in patients with SLE plasma miRNA expression. Upregulation of miR- 142- 3p and downregulation of miR- 155* in the plasma of SLE patients are new clinical diagnostic biomarkers to evaluate SLE activity.
