摘  要:目的探讨血清甲状腺激素(TH)、同型半胱氨酸(Hcy)水平联合测定在甲状腺功能减退诊治中的临床价值。方法分别检测125例临床甲状腺功能减退组患者、205例亚临床甲状腺功能减退组患者和50例正常体检对照者TH、Hcy水平,并进行统计分析。结果亚临床甲状腺功能减退组血清FT3、FT4水平与正常对照组相比,差异均无统计学意义(P>0.05),而血清TSH和Hcy水平明显增高,差异有统计学意义(P<0.01)。临床甲状腺功能减退组患者血清FT3、FT4水平明显低于正常对照组,而TSH、Hcy显著增高,差异均有统计学意义(P<0.01)。临床甲状腺功能减退组与亚临床甲状腺功能减退组患者比较,血清FT3、FT4明显降低,血清TSH和Hcy显著增高,差异均有统计学意义(P<0.01)。结论血清TSH、Hcy水平测定在亚临床甲状腺功能减退向临床甲状腺功能减退转化具有一定的诊断价值。Objective To investigate the diagnosis value of the combined detection of the thyroid hormone( TH) and homocysteine( Hcy) for patients with hypothyroidism. Methods To determine TH and Hcy levels in 205 patients with subclinical hypothyroidism,125 patients with clinical hypothyroidism and 50 controls,and the results were statistically analyzed. Results The serum FT3,FT4 levels in patient with subclinical hypothyroidism were compared with that of the controls,and the differences had no statistical significance( P〉0. 05),but serum TSH and Hcy levels increased significantly,with the difference statistically significant( P〈0. 01). The serum FT3,FT4 levels in patient with clinical hypothyroidism were significantly lower than the controls,while,TSH,Hcy levels increased significantly,and the differences were statistically significant( P〈0. 01). The serum FT3,FT4 levels in patient with clinical hypothyroidism were lower than those in patients with subclinical hypothyroidism,but TSH and Hcy levels were very significantly higher,and the differences were statistically significant( P〈0. 01). Conclusion The detection of TSH and Hcy levels was valuable for subclinical hypothyroidism translating to clinical hypothyroidism.
