血型系统中罕见B(A)亚型的鉴定及血清学特性分析

摘　　要：目的对罕见B(A)亚型进行血清学和基因定型,探讨该类血型患者的输血策略。方法采用试管凝集法鉴定血型,采用序列特异性引物-聚合酶链式反应法进行ABO基因分型,采用凝聚胺法进行交叉配血试验。结果试管凝集法检测3例样本定型为A弱B强,其血清中存在抗-A抗体;3例样本经血型抗原基因突变数据库比对,基因型均为B(A)亚型,其中1种样本为B(A)04/O01单体型,2种样本为B(A)04/O02单体型,3例样本均在第640位碱基发生了A>G的突变;3例样本与A、B、O、AB型供血者进行交叉配血,结果主侧或次侧不合,与O型、B型红细胞交叉配血结果相合,与AB型血浆交叉配血结果相合。结论罕见B(A)亚型的鉴定及血清学特性分析对后续患者的输血策略具有较大影响。Objective To conduct serological and gene identification on rare blood type B（ A） subtype and to explore the blood transfusion strategy on patients with it. Methods The ABO genotyping was performed by polymerase chain reaction with sequence specific primer- polymerase chain reaction,and the cross matching blood test was carried out by using the method of coagulation amines. Results All 3 samples were detected by coagulation amines as weak A strong B with antibody A in serum;they were compared with Blood Group Antigen Gene Mutation Database（ BGMUT）,with all three cases being B（ A） subtype,one of them was heplotype B（ A） 04 / O01,other two cases were heplotype B（ A） 04 / O02,all of them had a mutation at allele 640 A 〉G; the cross matching did not match the primary or secondary side of A,B,O,AB,but did match the red blood cell of type O and B,and with the plasma of blood type AB. Conclusion The identification and serological analysis of the rare B（ A）subtypes have a great impact on the blood transfusion strategies in the follow- up patients.
