小剂量右美托咪定滴鼻在普通胃镜中的应用

摘　　要：目的：探讨右美托咪定滴鼻在胃镜检查中的安全性和价值。方法选择80例拟行普通胃镜检查的患者，年龄30～60岁，体重45～65 kg，ASAⅠ－Ⅱ级。采用随机数字表法将患者分成两组（n=40）。A组操作前经鼻滴入0.5μg/kg右美托咪定，B组给予经鼻滴入适量生理盐水，滴鼻操作均在胃镜检查开始前25 min进行。胃镜检查前常规服用利多卡因胶浆行表面麻醉。分别记录两组患者T0-滴鼻前（基础值），T1-检查前（在此点两组患者分别经鼻滴入生理盐水和0.5μg/kg右美托咪定）、T2-胃镜置入即刻、T3-胃镜检查术中、T4-操作完毕时、T5-操作完毕后10 min各时间点的SBP（收缩压）、DBP（舒张压）、SPO2（血氧）、HR（心率）指标，视觉观察呼吸频率及呼吸变化并观察相关不良反应，评价患者舒适程度。结果两组T0时SBP、DBP、HR差异无统计学意义。A组T2、T3、T4时SBP、DBP、HR均显著低于B组（P＜0.05）。A组相关不良反应显著少于B组（P＜0.05）。A组患者舒适程度显著优于B组（P＜0.05）。结论右美托咪定可安全有效地应用于普通胃镜检查。Objective To investigate the safety and benefi ts of applying dexmedetomidine （Dex） intranasal for patients undergoing gastroscopy.Methods Eighty patients with ASAⅠ-Ⅱ, aged 30 to 60 years old and weight 45-65 kg requiring normal gastroscopy were randomized to 2 groups.Group A （n=40） received Dex 0.5μg/kg intranasally before operation, while group B （n=40） received normal saline. Nasal inhalation was performed 25 minutes ahead of operation. Lidocaine mucilage was routinely used for topical anesthesia before endoscopy. Systolic pressure （SBP）, diastolic pressure （DBP）, blood oxygen saturation （SPO2） and heart rate （HR） were recorded before induction （T0）, after induction （T1）, during intubation （T2）, immediatelyfi nished operation （T3） and 10 min after operation （T4）. The changes of respiratory rate and related adverse reactions were visually observed and the comfort level of patients were evaluated.Results SBP, DBP and HR at T2, T3 and T4 in group A were lower than those in group B （P〈0.05）. The incidences of adverse reactions in group A were signifi cantly lower than that in group B （P〈0.05）. The comfort level was better in group A than that in group B （P〈0.05）. Conclusion It is effective and safe for the use of Dex on patients undergoing normal gastroscopy.
