摘 要:目的探讨生长分化因子-15(GDF-15)对2型糖尿病肾病中的临床诊断意义。方法测定25名健康对照者和85名2型糖尿病患者的血清生长分化因子-15(GDF-15),分析其与尿微量白蛋白(m Alb)、尿素氮(BUN)、血肌酐(SCr)和血尿酸(UA)的相关性。结果大量白蛋白尿组、微量白蛋白尿组GDF-15水平均高于正常蛋白尿组,均具有统计学意义(P<0.05);相关性分析显示,GDF-15水平与BUN、SCr、m Alb水平呈正相关,与e GFR呈负相关,与UA无相关性;另以e GFR<90ml/min/1.73m2作为肾功能受损的诊断标准绘制受试者工作特征曲线(ROC),GDF-15和m Alb的曲线面积分别为0.795和0.726;根据cutoff值计算GDF-15和m Alb诊断肾功能受损的敏感度分别为:83.6%、81.7%,特异度分别为:64.5%、62.8%。结论血清生长分化因子-15(GDF-15)的检测在2型糖尿病肾病的早期诊断具有良好的临床价值。Objective To investigate the clinical diagnostic value of growth differentiation factor-15(GDF-15) in type 2 diabetic nephropathy. Methods Serum GDF-15 was measured in 25 healthy controls and 85 patients with type 2 diabetes mellitus,and urinary albumin(m Alb),urea nitrogen(BUN),serum creatinine(SCr) and serum uric acid(UA) were analyzed. Results In macroalbuminuria group and microalbuminuria group,GDF-15 levels were higher than that in normal albuminuria group,with statistically significance(P〈0.05). Correlation analysis showed that GDF-15 level was positively correlated with the level of BUN,SCr,m Alb,and negatively correlated with e GFR,without correlation with UA. In addition,we chose the e GFR90ml/min/1.73m^2 as renal damage diagnosis standard,the receiver-operating characteristic curve(ROC) was drawn. The area under curve of GDF-15 and m Alb were 0.795 and 0.726 respectively. For GDF-15 and m Alb,the sensitivity of the diagnosis of renal damage were 83.6% and81.7% respectively,and the specificity were 64.5% and 62.8% respectively. Conclusion The detection of serum growth differentiation factor-15(GDF-15) has good clinical value in the early diagnosis of type 2 diabetic nephropathy.
