同期MC3三尖瓣成形环植入治疗功能性三尖瓣反流的疗效分析

摘　　要：目的探讨MC3成形环植入治疗风湿性心脏病二尖瓣狭窄继发三尖瓣反流的近期疗效。方法回顾性分析39例风湿性二尖瓣狭窄继发三尖瓣反流二尖瓣置换同时行三尖瓣成形患者的临床资料,根据术中对三尖瓣不同处理方式分为两组:A组(n=19),二尖瓣置换同时行经典De Vega瓣环成形术;B组(n=20),二尖瓣置换同时行MC3成形环植入术。于术后3个月对患者进行心脏彩色超声心动图检查测量患者三尖瓣反流情况,比较两组间各项指标的差别。结果 A、B组都是有效的手术方法,但B组术后3个月三尖瓣反流改善效果好于A组(P<0.05)。结论二尖瓣置换术同时行MC3成形环植入成形安全有效,对患者术后近期右心功能的恢复有良好效果。Objective To investigate the efficacy of concomitant implantation of MC3 shaped ring in patients with tricuspid regurgitation secondary to rheumatic mitral stenosis. Methods A retrospective analysis of 39 cases suffering from functional tricuspid regurgitation secondary to rheumatic mitral stenosis was employed. All subjects were divided into 2 groups according to the treatment to tricuspid. Except for the uniform mitral valve replacement,patients in Group A（ n = 19） were performed additional tricuspid annuloplasty with MC3 shaped ring. While,the other subjects（ n = 20） underwent traditional De Vega procedure as control. Three months later,echocardiography examination was performed to compare the volume of right atrium,right ventricle and the tricuspid regurgitation between the two groups. Results A and B group were effective surgical method,but group B tricuspid regurgitation in postoperative 3 months was improved better than group A（ P 〈 0. 05）. Conclusion Concomitant MC3 ring implantation while mitral valve replacement is safe and effective for patients with tricuspid regurgitation,and is benificial to the recovery of right cardiac function in short term.
