多层螺旋CT单期排泄期尿路成像在泌尿系梗阻疾病的诊断价值

摘　　要：目的探讨多层螺旋CT单期排泄期尿路成像在泌尿系梗阻性疾病的诊断价值。方法经B超或其它临床资料证实具有泌尿系梗阻而原因不明的患者56例,进行单期排泄期CT尿路成像。对CT尿路成像及其X线辐射剂量进行分析评估。结果 56例肾盂输尿管疾病中,肾盂输尿管结石28例,先天畸形12例,泌尿系感染所致输尿管狭窄3例,尿路恶性肿瘤13例。拟进行的标准泌尿系CT尿路成像的平均X线辐射剂量为(15.22±4.54)mSv,而实际进行的单期排泄期尿路成像的平均X线辐射剂量为(9.43±4.36)mSv,二者有显著差别(t=15.36,P<0.01)。结论单期排泄期CT尿路成像在泌尿系梗阻性疾病的诊断中具有重要的临床应用价值,而且辐射剂量明显减低。Objective To evaluate the clinical value of multi-slice single excretory-phase CT urography in diagnosis of obstructing urinary tract disease. Methods Fifty six patients who were diagnosed with unexplained obstructing urinary tract abnormalities by B ultrasonic and other clinical data underwent multi-slice single excretory CT urography. CT urography and its Xray radiation dosage were analyzed. Results Among the 56 patients with renal pelvis and ureter disease,there were 28 cases of urinary tract calculi,12 cases of congenital anomalies,3 cases of ureter stenosis caused by urinary tract infection,and 13 cases of uroepithelial malignancies. The proposed standard mean X-ray radiation dosage of CT urography was（ 15. 22 ± 4. 54）mSv,and the mean X-ray radiation dosage of the excretory phase scan during actual test was（ 9. 43 ± 4. 36） mSv. There was significant difference between each other（ t = 15. 36,P〈 0. 01）. Conclusion Multi-slice single excretory CT urography plays an important role in diagnosis of obstructing urinary tract disease.
