ATP敏感性钾通道开放剂尼可地尔与二甲双胍合用对早期T2DN的治疗作用

摘　　要：目的观察腺苷三磷酸(ATP)敏感性钾通道开放剂尼可地尔与经典2型糖尿病肾病(T2DN)治疗药物二甲双胍合用,对早期T2DN的治疗作用。方法选取T2DN患者30例,分为对照组14例和试验组16例。对照组给予二甲双胍每天3次,每次0.25g,连续26周。试验组给予相同剂量二甲双胍合并尼可地尔每天3次,每次5mg,连续26周治疗。2组患者均在治疗前后分别检测空腹血糖、总胆固醇,三酰甘油,低密度脂蛋白、高密度脂蛋白、血尿素氮、血肌酐、尿清蛋白、白细胞介素-6(IL-6)、基质金属蛋白酶-9(MMP-9)水平。结果 2组患者治疗后空腹血糖水平对比差异无统计学意义(P>0.05);试验组患者治疗后血脂指标低密度脂蛋白水平显著低于对照组,差异有统计学意义(P<0.05);试验组患者治疗后肾功能指标血尿素氮、血肌酐、尿清蛋白水平均显著低于对照组,差异有统计学意义(P<0.05);试验组患者治疗后血清IL-6、MMP-9水平均显著低于对照组,差异有统计学意义(P<0.05)。结论二甲双胍联合尼可地尔可延缓T2DN病情进展。Objective To observe the therapeutic effect of ATP sensitive potassium channel opening agent nicorandil combined with classical treatment drug metformin for treating type 2diabetes nephropathy（T2DN）.Methods Thirty patients with T2 DN were selected and divided into the control group（14cases）and the experimental group（16cases）.The control group was given metformin 0.25 g,3times daily for 26 consecutive weeks.The experiment group was given the same dose of metformin and nicorandil 5mg,3times daily for 26 weeks.The fasting blood glucose,total cholesterol,triglycerides,low-density lipoprotein（LDL）,high density lipoprotein（HDL）,blood urea nitrogen,serum creatinine,urine albumin,IL-6and MMP-9levels before and after treatment were measured in both groups.Results There was no statistically significant difference in fasting blood glucose level after treatment between the control group and the experimental group（P〉0.05）;the LDL level after treatment in the experiment group was significantly lower than that in the control group with statistical difference（P〈0.05）;blood urea nitrogen,serum creatinine and urine albumin levels after treatment in the experimental group were significantly lower than those in the control group with statistical difference（P〈0.05）;the levels of serum IL-6and MMP-9after treatment in the experiment group were significantly lower than those in the control group with statistical difference（P〈0.05）.Conclusion Metformin combined with nicorandil could delay the progression of T2 DN.
