摘  要:目的探讨脑性瘫痪(以下简称"脑瘫")患儿年龄与加强型气管插管内径型号的相关性,为该类患儿全身麻醉时选择适宜型号的导管提供理论依据。方法选择2013年6月~2015年12月四川省八一康复中心择期行手术治疗的脑瘫患儿210例,ASA分级Ⅰ~Ⅱ级。按年龄段将患儿分为7组:〈1、1~〈2、2~〈3、3~〈4、4~〈5、5~〈6、6~〈7岁,每组各30例。麻醉诱导后,用视频喉镜暴露声门,评估声门的大小并参照公式"ID=2.5+0.4×年龄"选择相应型号的加强型气管插管,并备好相邻的型号。气管插管成功后连接呼吸机行机械通气。记录该患儿年龄与最终选择的导管内径值,分析二者的相关性。结果 210例患儿所插加强型气管插管的内径与年龄呈正相关(r=0.949,P〈0.01),回归方程为Y=2.100+0.415X,因此小儿加强型气管插管的内径选择可参考线性回归方程ID=2.1+0.4×年龄。结论在脑瘫患儿的全身麻醉中,患儿年龄与加强型气管插管的内径呈正相关,公式"ID=2.1+0.4×年龄"可能更适合脑瘫患儿加强型气管插管型号的选择。Objective To evaluate the correlation of age of children with cerebral palsy and internal diameter of strengthen endotraeheal tube, and provide theoretical basis for selecting appropriate type of strengthen endotracheal tube in general anesthesia. Methods From June 2013 to December 2015, 210 children with cerebral palsy in Sichuan 81 Rehabilitation Center scheduled for elective surgery were enrolled, all children with the American Society of Anesthesiologists (ASA) physical status Ⅰor Ⅱ. Children were divided into 7 groups according to age: 〈1, 1-〈2, 2-〈3, 3-〈4, 4- 〈5, 5-〈6 and 6-〈7 years, with 30 cases in each group. After induction of anesthesia, the glottis was exposed with wireless video laryngoscope, the glottal aperture was evaluated and the corresponding endotracheal tube was selected combined with smaller than traditional models according to the formula "ID = 2.5 + 0.4 ×age". After tracheal intuba- tion, mechanical ventilation was given after connected to a ventilator. The values of tube diameter and children's age were recorded, and the correlation between the two was analyzed. Results The correlation analysis showed that, children's age was positively associated with internal diameter of strengthen endotracheal tube (r = 0.949, P 〈 0.01). The regression equation was Y = 2.100 + 0.415 X, therefore, could be reference. Regression equation and the formula of ID = 2.1 + 0.4 × age could be used to choose the tube diameter. Conclusion In general anesthesia of children with cerebral palsy, the age and internal diameter of strengthen endotracheal tube show highly positive correlation, the formula "ID = 2.1 + 0.4 × age" may be more suitable for the selection of strengthen endotracheal tube in children with cerebral palsy.
