摘  要:目的探讨新生儿血清S100B蛋白、神经元特异性烯醇化酶(NSE)水平与新生儿低血糖脑损伤(HBD)的关系。方法选取2014年10月至2015年9月低血糖新生儿48例,依据临床表现分为症状性低血糖组(n=22)和无症状低血糖组(n=26)。正常对照组选取同期健康足月新生儿40例。各组新生儿留取出生后24、72 h血清样本,采用Human S100B Elisa kit及酶标仪检测血清S100B水平;应用罗氏411电化学发光免疫分析仪测定血清NSE水平。各组新生儿均行脑电图检查。结果症状性低血糖组血糖水平低于无症状性低血糖组和正常对照组(P<0.01),无症状低血糖组血糖水平低于正常对照组(P<0.05);症状性低血糖组低血糖持续时间高于无症状低血糖组(P<0.01);24 h血清NSE和S100B水平表达症状性低血糖组均显著高于无症状低血糖组和正常对照组(P<0.01),无症状低血糖组高于正常对照组(P<0.05)。而各组间72 h血清NSE和S100B水平差异无统计学意义(P>0.05);24 h血清NSE和S100B与血糖水平呈显著负相关性(P<0.05),与低血糖持续时间呈显著正相关性(P<0.05);血清NSE与S100B水平呈显著正相关性(P<0.05)。结论血清S100B蛋白及NSE在新生儿低血糖脑损伤时水平升高,且与患儿血糖降低程度以及低血糖持续时间相关,可作为反映新生儿HBD的早期敏感指标。Objective To investigate w hether the serum S100 B protein and neuron-specific enolase( NSE) levels w ere associated w ith neonatal hypoglycemic brain damage( HBD). Methods A total of 48 hypoglycemic neonates admitted during Oct. 2014 and Sept. 2015 w ere selected as the experimental group,w ho w ere divided into 2 subgroups according to clinical manifestations and EEG examination: symptomatic hypoglycemic group( n = 22) and asymptomatic hypoglycemic group( n = 26). Another 40 healthy subjects served as the control group. Blood samples w ere collected from all neonates 24 and 72 hours after birth. The levels of serum S100 B protein and NSE protein w ere measured w ith Human S100 B Elisa kit and Cobas 411 electro-chemiluminescence immunoassay analyzer,respectively. EEG examination w as performed after admission. Results The level of blood glucose in the symptomatic group w as low er than that of asymptomatic group and control group( P〈0. 01),and that of the asymptomatic group w as low er than the controlgroup( P〈0. 05). The duration of hypoglycemia w as longer in the symptomatic group than in the asymptomatic group( P〈0. 01). The symptomatic group had significantly increased levels of serum 24 h-NSE and 24 h-S100 B than the other 2 groups( P〈0. 01),and the asymptomatic group had higher levels than the control group( P〈0. 05). There w ere no statistical differences in the levels of serum 72 h-NSE and 72 h-S100 B among the 3 groups( P〉0. 05). The level of serum 24 h-NSE w as negatively correlated w ith 24 h-S100 B,positively correlated w ith the duration of hypoglycemia( P〈0. 05),and the serum NSE was positively correlated with S100 B protein( P〈0. 05). Conclusion The levels of serum S100 B protein and NSE elevate in neonatal HBD,w hich are closely associated w ith the severity and duration of hypoglycemia,suggesting that they may serve as early sensitive markers for the diagnosis of neonatal HBD.
