丹七软胶囊防治髋部骨折术后深静脉血栓临床观察

摘　　要：目的观察丹七软胶囊对髋部骨折患者发生深静脉血栓(DVT)的预防效果及对复发率、生存率的影响。方法将118例DVT阴性的髋部骨折患者分为两组。治疗组给予丹七软胶囊口服,对照组给予低分子肝素钠皮下注射,记录两组病例住院期间经超声诊断为DVT的例数,于入院用药前、用药1d、3d、7d后检测两组病例的血小板计数(PLT)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)与血浆D-二聚体(D-Dimer)的变化情况,观察两组病例的术中出血量与术后引流量,对两组病例进行随访,观察深静脉血栓(DVT)1年内的复发情况,并统计两组病例1年内的生存率。结果治疗组DVT发生率显著低于对照组(P<0.05);对照组PLT、PT、APTT变化程度更快更明显(P<0.05),治疗组D-Dimer变化程度更明显(P<0.05);治疗组的术中出血量与术后引流量显著低于对照组(P<0.05),治疗组的复发率显著低于对照组(P<0.05),治疗组的生存率显著高于对照组(P<0.05)。结论丹七软胶囊对髋部骨折后DVT有较好的预防效果,且减少了手术出血风险,安全性高,作用长久,可以有效降低DVT的复发率、提高患者的生存率。Objective To explore the preventive efficacy of Danqi soft capsule on deep vein thrombosis（ DVT） in patients with hip fracture and its effect on recurrence rate and survival rate. Methods 118 cases of hip fracture patients（ DVT negative） were randomly divided into observation group（ 56 cases） and control group（ 56 cases）. Observation group was given Danqi soft capsule orally,control group was treated with low molecular heparin subcutaneous injection. Record the cases of DVT diagnosed during the medication detect platelet count（ PLT）,prothrombin time（ PT）,activated partial thromboplastin time（ APTT） and plasma D dimer（ D- dimer） changes,observe intraoperative bleeding and postoperative drainage volume of both groups,analyze the 1 years survival rates. Results The incidence of DVT in observation group was significantly lower than that of the control group（ P〈0. 05）. PLT,APTT,PT,D- Dimer changes in control group were faster and more obvious（ P〈0. 05）. D- Dimer changed more obviously in treatment group; introperative bleeding and postoperative drainage volume of treatment group were obviously lower than control group（ P〈0. 05）,the recurrence rate of treatment group was obviously lower than control group（ P〈0. 05）,the survival rate of treatment group was obviously higher than control group（ P〈0. 05）. Conclusion Danqi soft capsule has good preventive effect on DVT after hip fracture,and can effectively reduce the risk of bleeding and decrease the recurrence rate to improve the survival rate of patients.
