剖宫产术子宫切口行连续套锁缝合及连续单层缝合的对照研究

摘　　要：目的比较分析连续套锁缝合及连续单层缝合在剖宫产术后子宫切口缝合的效果。方法将100例行剖宫产术的产妇按随机数字表法分为观察组和对照组,每组50例。对照组剖宫产术后子宫切口采用连续套锁缝合的方式,观察组采用连续单层缝合的方式。观察比较2组产妇住院时间、产后出血率、术后最高体温、术中出血量及手术时间等。所有患者术后均进行1年随访,对2组月经复潮时间及月经不调、切口假腔的发生率进行比较。结果观察组手术时间短于对照组[(48.46±7.35)min比(52.32±8.17)min,P<0.05]。2组住院时间、产后出血率、术后最高体温、术中出血量比较差异均无统计学意义(均P>0.05)。2组患者均随访1年,观察组切口假腔、月经不调发生率明显低于对照组(2.00%、8.00%比8.00%、20.00%,均P<0.05)。2组月经复潮时间比较差异无统计学意义(P>0.05)。结论在剖宫产术后子宫切口连续单层缝合能有效地减少手术时间,并降低月经不调及切口假腔发生率。Objective To compare the efficacies of continuous lock stitch and continuous singlelayer suture for closing uterine incision at caesarean section.Methods One hundred women undergoing caesarean section were randomly assigned to receive either continuous lock stitch（control group,n=50）or continuous single-layer suture（observation group,n=50）for closing uterine incision.The hospital stay,postpartum hemorrhage,highest postoperative temperature,intraoperative blood loss and operative time were recorded in both groups.The time to menstruation restoration and incidences of menstrual disorder and incisional false lumen were compared between the two groups after 1-year follow-up.Results The operative time in observation group was significantly shorter than that in control group[（48.46±7.35）minutes vs（52.32±8.17）minutes,P〈0.05）].There were no significant differences in hospital stay,postpartum hemorrhage,highest postoperative temperature,intraoperative blood loss and time to menstruation restoration between the two groups（P〉0.05）.After 1-year follow-up,incidences of incisional false lumen and menstrual disorder in observation group were significantly lower than those in control group（2.00%vs 8.00% and 8.00% vs 20.00%,respectively;P〉0.05）.Conclusion Continuous single-layer suture for closing uterine incision at caesarean section can effectively reduce operation time and incidences of incisional false lumen and menstrual disorder.
