手术室实施情景模式化护理的效果

摘　　要：目的探讨情景模式化护理在手术室中的应用效果。方法以在手术室进行全身麻醉手术的788例患者为观察对象,采用随机数字表法将其分为研究组和对照组,每组394例。对照组实施常规护理,研究组在对照组的基础上,实施手术室情景模式化护理。分别记录2组患者的手术时间及气管插管保留时间,以及进入手术室后5min(T0)、麻醉前1min(T1)及麻醉后1min(T2)的心率(HR)、动脉收缩压(SBP)、动脉舒张压(DBP)及血浆去甲肾上腺素(NE)水平,术后3d进行回访并问卷调查,统计2组患者对手术室护理的满意度。结果研究组患者T0、T1、T2时HR、SBP及NE水平均明显低于对照组(PT0分别=0.021、0.007、0.001;PT1分别=0.005、0.014、0.001;PT2分别=0.027、0.019、0.001)。研究组患者T0、T1时DBP明显低于对照组(PT0=0.033、PT1=0.027),T2时差异无统计学意义(PT2=0.093)。研究组患者对于手术室护理的总满意度明显高于对照组(P=0.013)。结论手术患者应用手术室情景模式化护理有利于维持患者血流动力学的稳定,并可提高患者对手术室护理的满意度。Objective To observe the efficacy of scene mode nursing in operation room.Methods A total of 788 patients undergoing general anesthesia were randomly given routine nursing alone（control group,n=394）or in combination with scene mode nursing（study group,n=394）.The operation time and endotracheal intubation retention time were recorded in both groups.The heart rate（HR）,systolic blood pressure（SBP）,diastolic blood pressure（DBP）and plasma norepinephrine（NE）level were measured 5 minutes after admission to operation room（T0）,1 minute before anesthesia（T1）and 1 minute after anesthesia（T2）.The patient satisfaction with nursing was investigated by questionnaire 3days after operation.Results The HR,SBP and NE level in study group were lower than those in control group at all time points（T0：P=0.021,0.007and0.001,respectively;T1：P=0.005,0.014 and 0.001,respectively;T2：P=0.027,0.019 and 0.001,respectively）.The DBP in study group was lower than that in control group at T0 and T1（P=0.033 and 0.027,respectively）.The difference in DBP was not significant between the two group at T2（P=0.093）.In addition,patient satisfaction with nursing in study group was higher than that in control group（P=0.013）.Conclusion Scene mode nursing can help to maintain the stability of hemodynamics and improve patient satisfaction with nursing in operation room.
