摘  要:目的探索1 064 nm波长皮秒(ps)脉冲Nd:YAG激光对乳猪皮肤组织的气化效应及气化区周围的热凝固效应。方法使用脉宽30 ps,频率10 Hz,最大单脉冲能量100 m J的1 064 nm脉冲Nd:YAG激光照射离体乳猪皮肤组织,按照单脉冲能量0、20、40、60、80 m J和100 m J,每次辐照时间分别为2、5 s和10 s进行分组,组合形成18组激光剂量,每组3个标本。使用超景深显微镜即时观察记录气化区深度及宽度。取受照皮肤组织制作组织切片,HE染色,显微镜下观察测量气化区外围凝固区宽度。根据所得数据绘制单脉冲能量及辐照时间与气化区宽度、深度及凝固区宽度的量效关系曲线,分析其作用规律及临床意义。结果气化区直径为740.66~1 455.32(1 168.85±188.57)μm,气化区深度106.44~1 314.80(412.16±289.84)μm;凝固区宽度65.35~238.73(141.10±47.68)μm。结论 1 064 nm波长30 ps脉宽Nd:YAG激光对乳猪皮肤组织有较强的气化作用,且气化作用的效应程度与单脉冲能量和每次辐照时间呈正相关趋势。用该激光气化目标皮肤组织时,对健康组织热凝固效应可控制在百微米级范围。Objective To investigate the vaporization effect and the surrounding thermal coagulation effect of the radiation of picosecond pulsed Nd: YAG laser at wavelength of 1 064 nm on suckling porcine skin tissue. Methods A 1 064 nm pulse Nd: YAG laser with a pulse duration of 30 ps and a frequency of 10 Hz was used to radiate suckling porcine skin in vitro. The porcine skin tissues were irradiated with different pulse energies( 20,40,60,80 and 100 m J) for different time periods( 2,5 and 10 s per time). So there produced 15 different lesioning points and 3 normal control points( 0 m J and 2,5 and 10 s endurance),and every point was repeated for 3 times. After irradiation,the width and depth of the evaporated zones of the lesioning points were measured with ultra-depth field microscopy. The coronal plane sections of every lesioning point were made into tissue slices,and then stained with hematoxylin and eosin. By the means of taking photomicrographs,the widths of the coagulation necrosis zones were measured. Finally,the curve of doses and effects was drawn. Results The average width of the evaporated zones of the lesioning points was 1 168. 85 ±188. 57 μm( 740. 66 ~ 1 455. 32),while the depth was 412. 16 ± 289. 84 μm( 106. 44 ~ 1 314. 80),and the average width of the coagulation necrosis zones was 141. 10 ± 47. 68 μm( 65. 35 ~ 238. 73 μm).Conclusion Picosecond pulsed Nd: YAG laser of 1 064 nm wavelength irradiates good vaporization effect on suckling porcine skin tissue,and the effect is positively correlated with the pulse energy and irradiation duration of every pulse. When the laser is evaporating target skin area,the caused thermal damage is limited in hundreds microns to the surrounding healthy tissues of the lesioning point.
