摘  要:目的对比超声引导与宫腔镜直视在宫内节育器(IUD)嵌顿取出术中的应用价值。方法选取2013年1月至2015年1月本院收治的IUD嵌顿患者132例为研究对象,将其随机分为对照组和试验组,每组各66例。对照组患者于宫腔镜直视下取环,试验组患者于彩色多普勒超声引导下取环。对比分析两组患者手术时间、术中出血量以及术后出血、感染、疼痛、子宫穿孔等术后并发症发生情况。结果试验组患者手术时间、术中出血量均明显少于对照组(P<0.05)。试验组患者术后并发症明显减少,其中术后出血、感染、疼痛的发生率均明显低于对照组(P<0.05)。结论超声引导下行嵌顿IUD取出术,可明显缩短手术时间,减少术后并发症,有利于对断裂后完全嵌入肌层而不显露的IUD的精准定位,提高IUD取出术的准确性和安全性。Objective To compare the application between under hysteroscopy and ultrasound guidance embedded itrauterine device removal. Method From January 2013 to January 2015, 132 patients diagnosed IUD embedded in our hospital were selected as the research objects, and they were randomly divided into control group and test group, 66 cases in each group. Patients in control group were treated with hysteroscopy guided IUD removal, and patients in test group were treated with color Doppler ultrasound guided IUD removal. The operation time, intraoperative amount of bleeding and the occurrence of postoperative hemorrhage, infection, pain, uterine perforation and other complications between the two groups were compared. Result The operation time and intraoperative amount of bleeding of test group were significantly lower than control group(P 0.05). The complications of test group were significantly decreased(P 0.05), and the postoperative hemorrhage, infection and pain of test group were significantly lower than control group(P 0.05). Conclusion Ultrasound guided IUD removal can significantly shorten the operation time and reduce the postoperative complications. This may help to accurate location of unexposed IUD, as a result, the safety of the operation will be improved.
