地佐辛联合咪达唑仑在行冠状动脉搭桥术患者麻醉前镇静中的应用价值

摘　　要：目的探讨地佐辛联合咪达唑仑在行冠状动脉搭桥术患者麻醉前镇静中的应用价值。方法选取2013年8月至2014年6月河南省胸科医院收治的100例择期行冠状动脉搭桥术患者作为研究对象,按随机数字表法将其分为研究组和对照组,各50例。研究组患者静脉注射地佐辛与咪达唑仑,对照组进行单纯地佐辛麻醉,比较两组患者给药后5 min(T_1)、动脉穿刺后(T_2)、给药后30 min(T_3)的心率(HR)、平均动脉压(MAP)、中心静脉压(CVP)、动脉血氧饱和度(SaO_2)及术后镇静效果。结果 T_1、T_2、T_3时,研究组患者的HR、MAP、CVP均明显低于对照组,SaO_2明显高于对照组,差异均有统计学意义(均P<0.05);研究组患者的清醒时间、睫毛反射消失时间均明显短于对照组,Ramsay评分明显低于对照组,差异均有统计学意义(均P<0.05)。结论麻醉前给予行冠状动脉搭桥术患者咪达唑仑与地佐辛,患者生命体征平稳,镇静镇痛效果较佳,术后苏醒快。Objective To explore dezocine combined with midazolam in patients undergoing coronary artery bypass grafting with midazolam anesthesia sedation before the application.Methods August 2013 to 2014 June Henan chest hospital from 100 patients undergoing coronary artery bypass grafting in patients with selected as the research object,according to the random number table method can be divided into the study group and control group,50 cases in each. Study group were treated with intravenous injection of dezocine and midazolam,control group simple dezocine anesthesia,were compared between the two groups after administration of 5 min（T_1）,after the arterial puncture（T_2）,to drug after 30 min（T_3）,heart rate（HR）,mean arterial pressure（MAP）,Central venous pressure（CVP）, pulse oxygen saturation（SaO_2） and postoperative sedative effect.Results At T_1,T_2 and T_3,patients in the study group,HR,MAP,CVP were significantly lower than those of the control group,Sa O2 was significantly higher than that of the control group,differences were statistically significant（P〈0.05）;patients in the study group awake time,eyelash reflex disappearance time were significantly shorter than that of the control group,Ramsay score was significantly lower than that of the control group,differences were statistical ly significant（P〈0.05）.Conclusion Before anesthesia to give the patients undergoing coronary artery bypass grafting Mi of midazolam and dezocine,stable vital signs of the patients, sedative and analgesic effect is better,postoperative faster recovery.
